| Objective:MSCT is the main imageology in diagnosing the staging of preoperative advanced gastric carcinoma and the rate of detecting regional lymph nodes and distant metastasis is not high.But PET/CT has very well advantages in the two facts.So the studing of comparing 18F-FDG PET/CT with MSCT in the preoperative advanced gastric carcinoma is very useful.Methods:Review advanced gastric carcinoma patients treated by operation in the first affiliated hospital of Guangxi Medical University from July 2006 to December 2008,abstract 39 patients taking PET/CT and abstract 40 patients performing MSCT before operation,diagnose the TNM staging by respective criteria and compare with the operation histopathology.SPSS13.0 statistical package was used to analyze relation between PET/CT and MSCT in this study.P<0.05 was considered statistically significant.Results:(1)For diagnosing primary lesions,the PET/CT scan is accurate in 92.3%compared with 85%from MSCT,the comparison has no statistical significance(P>0.05).(2)For diagnosing regional lymph nodes,the PET/CT scan is accurate in 66.7%compared with 50%from MSCT,the comparison has no statistical significance(P>0.05) For diagnosing N3 lymph nodes,the PET/CT scan is accurate in 100%compared with 62.5%from MSCT,the comparison has statistical significance(P<0.05)(3) For diagnosing distant metastasis,the PET/CT scan is accurate in 97.4%compared with 75%from MSCT,the comparison has statistical significance(P<0.05)Conclusion:(1)PET/CT and MSCT have high accuracy in diagnosing primary lesions of advanced gastric carcinoma,there is no difference in their detecting.(2)PET/CT and MSCT have accuracy in diagnosing regional lymph nodes of advanced gastric carcinoma,there is no difference in their detecting.But for detecting the N3 lymph nodes,PET/CT has higher accuracy than MSCT.So PET/CT may be a way to detect if advanced carcinoma has micrometastasis.(3)PET/CT has higher accuracy in diagnosing distant metastasis of advanced gastric carcinoma than MSCT.(4)For the false negative,PET/CT combining abdominoscopy may decrease or avoid exploratory laparotomy. |
