Purpose To find differential expression protein by proteomic technology in multiple myeloma(MM) and healthy people control group, while establish diagnostic models of primary and study their clinical diagnostic application value and prospect.Methods Serum protein from 30 patients with multiple myeloma at preliminary diagnosis(MM group) and 33 healthy population control group were analyzed by surface-enhanced laser desorption-ionization time-of-flight mass spectrometry(SELDI-TOF MS) and CM10 protein chip array,and collected data by Ciphergen Proteinchip3.0 version of the analysis software. Biomarker Wizard software was used to identify proteomic features. Biomarker Patterns3.2 Software set up a diagnosis model and evaluated its sensibility and specificity.Results The analysis of the serum protein map showed, there were 57 protein peaks,in which there was difference between two groups (p<0.05) in 30 protein peaks. Compared with the control group, 10 protein peaks were up-regulated expression and 20 protein peaks were down-regulated expression in MM group. Using three differential protein peaks M/Z4903.00,4971.15,5343.98, to build a diagnostic model, then check by validation, we found that the sensibility of this model was 86.67%(26/30),specificity was 81.82%(27/33), positive likelihood ratio was 4.77, negative likelihood ratio was 0.16, positive predictive value was 81.25%(26/32), negative predictive value was 87.10%(27/31). Conclusion SELDI-TOF-MS Protein Chip technology is a quick, easy and convenient, and high input analyzing method which can screen several relatively specific protein markers from the blood serum of MM patients. The diagnosis model which comprising of 3 protein peaks with mass-to-charge ratio of 4903.00,4971.15,5343.98 can obtain high sensibility and specificity. So the diagnosis model has significant importance in diagnosis, especially diagnosis of MM.Novelty of this project Using the high input of SELDI-TOF-MS in analyzing the differential protein of the blood serum of MM patients, paying close attention to the small molecular weight, then got the 3 protein peaks with mass-to-charge ratio of 4903.00,4971.15,5343.98 to build a diagnosis model, which had significant value in diagnosis of MM.
