Clinical Epidemiological Analysis For Aneurysmal Subarachnoid Hemorrhage In Shijiazhuang 2003-2007

Objective:Prognosis of patients with aneurysmal subarachnoid hemorrhage(aSAH) is effected by many factors.The article analyzed prognostic factors in sex,age,Hunt&Hess Classification, location and amount of aneurysms,time and methodes of operation,hyper-tension and smoking for subarachnoid hemorrhage.To investigate the main factor on prognosis of aneurysmal Subarachnoid Hemorrhage( aSAH ) as so to provide theoretic foundation to treat intracranial aneurysm.Methods: The clinical data of 794 cases with aSAH was studied retrospectively in seven Shijiazhuang,s hospital during 2003-2007; We analyzed prognostic factors in sex,age, Hunt&Hess Classification, location and amount of aneurysms, time and methodes of operation, hypertension and smoking for subarachnoid hemorrhage. Singular factor analysis used X2 Test.Logistic regression,using stepwise programme to get discriminant factors and equations at 0.05 level,was applied to analyze risk factors related to prognosis of aSAH.Results: There were 866 intracranial aneurysms in 794 cases,of them 61 patients had multiple aneurysms,51 patients with two aneurysms and 10 patients with three or over aneurysms.There were 292(33.72%) aneurysms in the anterior communicating artery(AcoA), 317(36.61%) aneurysms in the posterior communicating artery(PCoA), 9(1.04%) in the ophthalmicartery (OA); 61(7.04%)in the internal carotid artery (ICA), 114(13.16%) in the middle cerebral artery(MCA), 15(1.73%)in the anterior cerebral artery(ACA),43(5.43%) in the vertebral- basilar artery(V-BA). 569( 71.66) cases had a favourable outcome and 225(28.34%) were severely.The overall incranial aneurysms mortality was 6.68%.Univariate analysis showed that there was statistical significance of difference in Hunt-Hess grade (P<0.0001), age(P<0.0001) and hypertension (P=0.0226); Multivariate logistic regression analysis showed that Hunt-Hess grade(P<0.0001,OR=34.854), age(P<0.0001,OR=1.779) and hypertension (P=0.057,OR=1.413)were related to prognosis of aSAH.While it was not related to the gender,time and methodes of operation, location and amount of aneurysms and smoking.Conclusion: 60.71% of IA patients were at age of 40 to 60 years and the peak at age of 50-55 years;Hunt-Hess grade,age and hypertension are independent risk factors for the prognosis of aSAH; The prognosis of a SAH patients with more Hunt-Hess scores and age of 60 and overis poorer.
