Acute coronary syndrome is a kind of disease which is associated with acute myocardial ischemia, and it is usually caused by coronary artery disease and it will increase the possibility of cardiac sudden death; acute coronary syndrome is composed of spectrum of disease caused by acute ischemic or necrotic including unstable angina, ST elevated myocardial infarction, non-ST elevated myocardial infarction and cardiac sudden death. Percutaneous coronary intervention (PCI) is a treatment which can prevent stenosis and improve the blood supply of coronary arteries by expanding the coronary artery with all means of duct expansion.As an important medicine in the treatment of patients who suffer from acute coronery syndrome, especially for those who are under the treatment of percutaneous coronary intervention (PCI), clopidogrel has been used widely. However, some clinical researches proved that parts of patients have resistance to clopidogrel.Scholars analyze clopidogrel resistance from many aspects, but they have not found any effective way to solve this problem. Through the summary to a number of studies on clopidogrel resistance, this article affirmes the results and clinical significance in this field, and proposes seven problems which should be solved in the future study. Systematic study on clopidogrel resistance should be done from multilayers and relations among other factors in acute coronary syndrome.
