Analyze The Relative Factors On The Metastasis Rules Of The Lymph Node In The Advanced Colon Carcinoma

Objective:To study the relationship of the age,circumference of penetration ,the site of original tumor and the local infihraton of colon carcinoma with lymph node metastasis,in order to provide a basis for better surgical therapy for patients with colon cancer.Methods:The metastasis of the lymph node in 108 cases of advanced colon cancers from October of 2007 to October of 2008 in our hospital were analyzed retrospectively.Results:Of 108 cases,52 cases(48.2%)showed LN metastasis.In a total retrieved and examined 1202 lymph nodes,206 lymph nodes(17.14% )showed metastasis.In three groups of the peiicolon lymph nodes (Group 0～5 cm,Group 5～10 am,and Group >10 cm respectivelv from the tumor) .The rate of node metastasis in patients aged below 40 was greater than that in the other groups(P<0.05).The lymphatic metastasis rate in Right hemicolon was higher than that in Left hemicolon(P < 0.05). depth of tumor invasion.The lymphatic metastasis rate in poorly differentiated colorectal cancer was higher than that in well differentiated(P < 0.05).The metastatic rate of Nl,N2,and N3 was 14.8%,7.4%,3.7% in Group T2;52.4%,21.4%,9.5% in Group T3;The LN metastasis of and 61.1%,30.5%,13.9% in Group T4. Respectively,there was significant difference among the group T2,T3,and T4(P<0.0 1).Conclusion:Lymphatic metastasis of advanced colorectal cancer is associated with age,depth of invasion,circumference of penetration and tumor site.The gender was not correlation factor of lymphatic metastasis.The metastasis of pericolon lymph nodes is mainly in those nodes located within 0～5 cm of the tumor,but rarely in those nodes located more than 10 cm from the tumor.The metastatic rate of lymph node in right colon cancer is significantly higher than that in left colon cancer.D3 radical resection is necessary for colon carcinoma and dissection of the central lymph nodes shouldespecially be carried out,especially for the tumour serosal invasion,poor differentiation ,we should clear up the nodes in N3.The length of excised bowel should be 10 cm above and below the tumor.
