Objective To investigatethe cognitive profiles of mild cognitive impairment due to subcortical small vessel disease (MCI-SSVD) and mild cognitive impairment caused by Alzheimer's disease (MCI-AD) and to find out the characteristic proton MR spectroscopy (1H MRS) profiles of MCI.Methods (1)The auditory verbal learning test, mini mental state examination(MMSE), and Montreal cognitive assessment(MoCA) were performed on 23 MCI-SSVD patients,18 MCI-AD patients, and 14 healthy controls. The impaired domains in patient groups were determined. (2)Two single-voxel sequences (TR1,5000ms, TE 30 ms) were carried out in the both hippocampi of 10 MCI patients and 10 healthy controls, and the ratios N-acetylaspartate (NAA)/creatine (Cr), choline (Cho)/Cr, myo-inositol (MI)/Cr, NAA/MI and NAA/Cho were compared statistically.Results (1) The auditory verbal learning test inmediate recall,delayed recall, mini mental state examination(MMSE), Montreal cognitive assessment(MoCA) were significantly worse in the two patient groups than the healthy controls. Compare with the healthy controls, MCI-SSVD patients did worse in the delayed recall, digital span, attention, sentence repeat and abstract which are parts of the MoCA, while MCI-AD patients did worse in the sentence repeat and delayed recall of the MoCA.MCI-AD patients did worse in the delayed recall than MCI-SSVD patients. (2) NAA/Cr ratio of the right hippocampi was lower in in MCI patients than in the healthy controls, while MI/Cr ratios of the both hippocampi and MI/NAA ratios of the both hippocampi were higher. There is no obvious difference of Cho/Cr ratios of the both hippocampi between MCI patients and the healthy controls. The negative correlation between MoCA and MI/Cr ratios of the both hippocampi and the negative correlation between MoCA and MI/NAA ratios of the both hippocampi were found.Conclusions Our research found that both MCI-SSVD and MCI-AD have a multiple- domain cognitive deficit,with memory impaired most seriously. MRS can ben used to differetiate MCI from normal aging.
