Objectives: The purposes of our study is to qualitatively examine NMO-IgG by indirect immunofluorescence assay and compare the sensitivity and specicity of the NMO-IgG testing method in Chinese people. With the observation of the correlation between NMO-IgG and the clinical features of CNS demyelinating diseases, we can provide a valuable and easy-testing laboratory marker for the diagnosis and differential diagnosis.Methods: 1,Patients:The patients who presented with CNS demyelinating diseases and those with other neurological diseases in the First Affiliated Hospital of Fujian Medical University between March 2008 and January 2009 were studied.It included 22 clinically ascertained NMO patients(Group A) , 26 CIS patients (GroupB) and 51 additional control patients with classic multiple sclerosis 31 and miscellaneous neurological disorders 20. All sera taken from the patients who didn't have breakfast were stored at 4℃refrigerator for use.2,The test of NMO-IgG :All sera were sent to Shanghai Huashan Hospital of Fudan University and determined by indirect immunofluorescence assay.They prepared the standard mouse brain stem and cerebellum (which could express the aquaporin -4) into 10um slices, fixed by 4% formaldehyde then kept in -70℃refrigerator;Thus,the diluted sera were dropped on the brain slices then disposed by fluorescence-labeled anti-human IgG and examined by the fluorescence microscope.3, Analysis of the correlation between NMO-IgG antibody and clinical features :⑴calculation of the positive rate of NMO-IgG in each group and analysis of its sensitivity and specificity for the diagnosis of NMO;⑵comparison of the clinical features of NMO (onset of age, sex, initial symptoms, the severity of disease, etc).4, Statistical analysis: the SPSS10.0 software, chi-square test Results:1,22 NMO cases have 12 of NMO-IgG positive, in 26 cases of CIS has 9 cases of patients with NMO-IgG Positive, 31 cases of MS in 3 cases of patients with NMO-IgG positive, 20 cases of other non-demyelinating disease neurology patients Who has 0 cases of NMO-IgG positive. Positive incidence of the NMO-IgG: NMO group was 54.55%, CIS group 34.62%, MS group 9.68%, others 0% (P <0.05).2, Evaluate the sensitivity and specificity of NMO-IgG in NMO diagnosis. Conclusion:1, The test of the NMO-IgG by indirect immunofluorescence assay is much more sensitive and specific in the diagnosis of NMO .2, NMO- IgG can be regarded as a specific biological marker in the differential diagnosis between MS and MNO.
