The Iconographical Diagnosis And Prognosis Evaluation In The Acute Phase Of Mesencephalon Injury

Objective: To offer a reference of diagnosis and prognosis to patients with mesencephalon injury by using MRI and MRS within 7 days. Methods: Get the patients with mesencephalon injury from Nov.2007 to Sep.2008, and acquire as much patients without even the cortex injury and a half people without injury to build a group of contrast. Analyzing the data of cMRI, MRS, Evoked potential and following up referred patients 6 months, we do a comparison by evaluating the patients'life quality through GOS and Barthal score. And we could offer a reliable prognosis according to the outcomes.Results: After comprehensive research of history, clinical manifestation, iconographical feature and, the most important, the decomposition of the MRS data, the following results gradually emerge. The MRI shows the relationship between the pars affecta and prognosis as follows (mesencephalon, better to worse): unilateral lateral part>bilateral lateral part >ventrosuperficial part>dorsal part. Meanwhile, if the NAA peak is smaller than the creatine peak, then that the patient's central nervous system was badly damaged is discovered. And we find that the upgraded level of brain stem auditory evoked potential shows a bad outcome.Meanwhile, a long latent period and decreased amplitude of visual evoked potential recover a bad consequence.Conclusion: Through data at hand, we could acquire iconographical classification, diagnosis and law of prognosis. With the information, a reliable diagnosis and prognosis evaluation could be made to patients with mesencephalon injury.
