| Objective To evaluate and compare the diagnostic value of CTA and 2D DSA,3D DSA for intracranial aneurysms.Methods In 92 patients with clinical manifestations suggestive of harboring intracranial aneurysms who underwent all examinations of routine CTA,2D DSA and 3D DSA,were involved in the study,77 cases of them were diagnosed with intracranial aneurysm,and totally 101 aneurysms were found.The display ability of CTA,2D DSA and 3D DSA for the 101 aneurysms was retrospectively analyzed and assessed by two neuroradiologists blindedly,which contains aneurysm detective rate,and visibilities of both aneurismal neck and its relation to parent artery according to its location and size (dividing into two groups with a diameter≥3mm and<3mm).Results①In terms of aneurysm detective rate,no significant statistical difference (P=0.250) was found between aneurysm detective rates of 2D DSA and CTA with a diameter≥3mm;while significant difference(P=0.008) was found between those of CTA and 2D DSA in the detection rate of aneurysm with the diameter<3 mm;and there was no significant statistical difference(P=0.078) between aneurysm detective rates of 2D DSA and CTA totally.②With regard to display of aneurismal neck and its relation to parent artery,no significant statistical difference(P>0.05) was found between 2D DSA and CTA with a diameter≥3mm or<3mm;while significant difference(P<0.05) was found between 3D DSA and CTA.③According to the analysis of 22 anterior communicating artery aneurysm,no significant statistical difference(P=0.119) was found between 3D DSA and CTA with display of anterior communicating artery aneurismal neck and its relation to parent artery;while significant difference(P=0.000) was found between 2D DSA and CTA, although no significant statistical difference(P=0.249) was found between 2D DSA and CTA with aneurysm detective rate.④According to the analysis of 32 posterior communicating artery aneurysm,no significant statistical difference(P>0.05) was found between 2D DSA and CTA with aneurysm detective rate and display of aneurysmal neck and its relation to parent artery; while significant difference(P<0.05) was found between 3D DSA and CTA with display of aneurismal neck and its relation to parent artery.⑤According to the analysis of 22 internal carotid artery aneurysm,significant statistical difference(P<0.05) was found between 2D DSA and CTA with aneurysm detective rate;and no significant difference(P>0.05)was found between 2D DSA and CTA with display of aneurismal neck and its relation to parent artery;while significant difference (P<0.05) was found between 3D DSA and CTA with display of aneurismal neck and its relation to parent artery.Conclusion①2D DSA shows no significant difference to CTA in aneurysm display rate and the display of aneurismal neck,and its relation to parent artery with a diameter≥3mm.②2D DSA is significantly advantageous over CTA with a diameter<3mm, while 2D DSA shows no significant difference to CTA in the display of aneurismal neck,and its relation to parent artery.③CTA shows no significant difference to 3D DSA in the display of anterior communicating artery aneurismal neck,and is significantly advantageous over 2D DSA.④2D DSA is significantly advantageous over CTA in internal carotid artery aneurysm display rate,while 2D DSA shows no significant difference to CTA in the display of aneurismal neck,and its relation to parent artery.⑤3D DSA has more advantages in aneurysm display rate and the display of aneurismal neck,and its relation to parent artery than CTA,especially for posterior communicating artery aneurysm and internal carotid artery aneurysm. |
