Objective To investigate the relationship between the pathology and prognosis of gastric cancer, through the summary of patient's age, gender, preoperative Hb value, the value of preoperative Alb,surgery and the pathological conditions, including tumor infiltrati on depth, tumor differentiation, site, size, number of metastatic lymph node, the total number of examined lymph nodes and the ratio between metastatic and examined lymph nodes(N ratio).Patients & Methods From January 2004 to June 2008,143 gastric carcinoma cases were confirmed the preoperative pathology and radical surgery feasible by the medical team, through searching hospital records and telephone follow-up study to obtain imformation.A retrospective analysis of patients with gastric cancer in this group age, gender, preoperative Hb value, the value of preoperative Alb,surgical and pathological conditions(including tumor infiltrati on depth, tumor differentiation, site, size, number of metastatic lymph node, the total number of examined lymph nodes and the ratio between metastatic and examined lymph nodes(N ratio)) affecting the prognosis of patients with gastric cancer was done, and then the relationship between of the pathological factors was analyzed through the application of SPSS software. Results in 143 cases of follow-up information on survival of gastric cancer patients, male to female ratio is 1.92:1,≥60-year-old patients with 87 cases(60.8%), 60-year-old middle-aged and young patients with 56 cases(39.2%),the overall survival rate between the two groups is similar.62 cases(43.4%) of anemia and 14 cases(9.8 %) with hypoalbuminemia in 143 cases were compared with no anemia or hypoalbuminemia, separately. The overall survival rates of the different groups is significant different(p<0.05).The overall survival rate of 73 cases in distal gastric gastrectomy goup is significantly higher than in the total and proximal gastrectomy group.Univariate analysis showed that tumor invasion, tumor differentiation, tumor location, tumor diameter, number of metastatic lymph node, the total number of examined lymph nodes and the ratio between metastatic and examined lymph nodes(N ratio) were influencing prognostic factors for survival. The ratio between metastatic and examined lymph nodes was closely related with the tumor infiltrati on depth, tumor differentiation, site, size of the cancer(p<0.05).Conclusion There were the following factors impacting on survival prognosis of gastric cancer:the value of preoperative Hb,preoperative Alb values and pathological factors, including tumor infiltrati on depth, tumor differentiation, site, size, number of metastatic lymph node, the total number of examined lymph nodes and the ratio between metastatic and examined lymph nodes(N ratio). The staging system based on the total number of examined lymph nodes and the ratio between metastatic and examined lymph nodes(N ratio) is more predictable than the traditional staging system for the prognosis of gastric cancer.
