Objective:To investigate the control rate and the medical cost of the diabetic patients after the intervention with blood glucose,lipids,blood pressure under the current health care model.Methods:A retrospective study was performed observing 123 cases of diabetic patients selected form the diabetes management database,who had been followed up for more than one year.Retrieving blood glucose,lipids,blood pressure data before and after the treatment from the database.Calculating the metabolic control rate,the drug expenditure,and the examination cost of the whole year followed by the diabetes screening path.Results:The fasting plasma glucose,postprandial blood glucose,glycosolated hemoglobin A1c(HbA1c),high-density lipoprotein(HDL-C) levels get improved after the treatment,having statistically significant difference(P<0.05).There are 73 cases (59.35%) reach the blood glucose control(HbA1c≤6.5%),while for the blood pressure control goal(BP≤130/80mmHg) 15 cases(50%) in the group of the diabetes patients with hypertension,and the lipid control(LDL-C≤100mg/dl)are 19 cases(42.2%) in the group of the diabetes patients with hyperlipidemia.The all controlled case are 23(18.70%).The average cost of the regular screening is 1549.0 Yuan a year for a single patient,while the total medical expenditure is 6307.1 Yuan, including 4897.9 Yuan for diabetes,2613.6 Yuan for hypertension among the patient with hypertension,and 1928.0 Yuan for hyperlipidemia among the patient with hyperlipidemia.Among the patients reaching the blood glucose control goal,the medical expenditure is 4615.3 Yuan to lower 1%HbA1c for lowering blood glucose.Conclusion:Although the standard management of diabetes patients helps to improve the control rate of blood glucose,lipids and blood pressure of diabetes patients,it isn't satisfying.The current medical expenditure is expensive for controlling the metabolic disorders.The aim of this study is lower the incidence of long-term complications by controlling the blood glucose,lipids,blood pressure.The saving in medical expenditure of complications can result in improving the efficiency.
