Objective: To evaluate the clinical result of complex hip arthroplasty using Zweymüller cementless biconical cup. Methods: From october 2005 to November 2008, 30 hips in 26 patient with developmental hip dysplasia or component loosening after total hip replacement who were treated with cementless total hip arthroplasty were followed up. There are 5 males(5 hips) and 21 females(25 hips) with an average age of 55.2 years(rang from 36 to 77 years). Among the total, 19 cases(23 hips) were diagnosised DDH , 7 cases(7 hips) were diagnosised component loosening resulted from total hip replacement. Harris Hip score system was applied. Result: All cases accepted the follow up services, the time differed from 6 to 40 months, the average time is 24.1 months. Harris Hip score, increaseed from preoperative 35.83 point to postopertive 88.25 point. Due to infection, one acetabulum cup loosening after revision operation, no other patients felt uncomfortable and pain. The x-ray show that the location of acetabulum component and femoral stems is satisfactory, no infection or loosing signs occure, acetabular heterotopic bone formation, all patients can walk without walking stick.The fineness is 83.33%. Conclusion: The application effect of Zweymüller cementless biconical cup on complex hip replacement received good effects on the hip function recovery procedure, bone cement is unnecessary in the operation, the clinical effects are satisfactory. The adequate preoperotive preparation, the full comprehension of the acetabulum defect styles and well-trained surgical operation technique, as well as the complete rehabilitation training after the operation are the key to the successful.
