| Objective:To research the significance of videokymography for vocal fold vibration in diagnosis of voice diseases.Methods:①The voice samples were collected from 56 patients with voice disorder and 26 subjects with normal voice.There were 24 vocal cord polyp patients,10 vocal nodules patients and 22 chronic pharyngitis patients.Chose 26 subjects with normal pronunciation,non-throat diseases,non-surgical history,non-respiratory system diseases,non-digestive system diseases,non-nervous system diseases,non-alcoholic drinks and non-tobacco habits as control group in the same period.②Used Germany XION electronics laryngoscope and voice acoustic detection equipment supporting XION got patients' electronic video and voice acoustic parameters.Got VKG images of each member and analyzed their characters.Surveyed all subject voice handicap index and got the score of every part.Vocal cord polyps patients and vocal nodules patients were admitted to hospital purposes under the microscope under general anesthesia surgery.Specimens were sent to pathology.Vocal cord polyp and vocal nodules were labeld on the bases of pathology result.Retested their voice acoustic parameters one month later.Results:①There are statistically significant difference of the lesion groups' acoustic parameters increased when compared with normal control group(P<0.05).②The voice parameters of vocal cord polyps patients and vocal nodules patients after surgery decreased with statistically significant differences(P<0.05).③Different diagnostic groups' largest open phase time value(T)have a statistically significant difference (P<0.05).④Time periodicity index(TPI)between individuals has a statistically significant difference(P<0.05).⑤There was a significant correlation between the closing angle(β) and the opening angle(α) of VKG images recorded(P<0.0001);The largest open phase time value(T) is related to the diagnosis with a negative correlation between packet(P<0.05).⑥There are significant difference between normal group and chronic pharyngitis group,vocal polyp group and chronic pharyngitis group as well(P<0.05).Theβof group vocal nodules and chronic laryngitis has statistical significance(P<0.05).The VHI score of female who has vocal cord polyp or chronic laryngitis is higher than males' who has the same disease(P<0.05).⑦Under normal circumstances there are no statistically significant difference betweenβandα(P>0.05).⑧Every part of VHI score(P,E,F)has no relation with jitter and shimmer.⑨The VHI score of polyp group of male,compared with normal male control group,was significantly higher with statistically significant differences(P<0.05).⑩Each female patients group's VHI score was higher and statistically significant difference when compared with the normal female control group(P<0.05).(11)The VHI score between male control and female control group has no significant difference(P<0.05).(12)The female'sVHI score who has vocal cord polyp or chronic laryngitis is higher than males' who has the same disease(P<0.05).Conclusion:When there are some pathological changes about the vocals,the stable of the vocal changed as well,which can behave as jitter and shimmer increased.They have some detection rate on vocal cord polyps,vocal nodules,and chronic laryngitis.They can be used as the evaluation of the treatment-efficacy of target diseases.Under normal circumstances,vocals move almost symmetrically,not only in the range of left and fight side but also the top and the basic of the same side.When there are some pathological changes on the vocal,the mucous membrane's flexibility changed simultaneously which can be seen in the VKG image characters' difference. These differences can induce voice change.Analyzing for VKG images can help to diagnose voice diseases.The VHI score between male control and female control group has no significant difference.For female,it is easier to have psychological impact on them when they have voice diseases.Every part of VHI score(P,E,F)has no relation with jitter and shimmer,they should not substitute for each other.As an objective methods of voice detection VKG images is more intuitively and vividly.Practicely,each clinical detection methods focus on a different meaning.We should combine a variety of detection means and conduct a comprehensive analysis from various angles. |
