Objective Study relationship between diameters of portal vein system, size change of liver and spleen,liver cirrhosis and liver function change scored by MELD by measuring the diameters of portal system,liver size and spleen size in liver cirrhosis patients,observing portal-body collateral circulation changes.Method 109 cases of patients confirmed cirrhosis by clinical data, Laboratory,B-USG and pathology.46 cases for liver function MELD scored equal or less than 15,42 cases for MELD scored between 15-25,21 cases for MELD scored higher than 25 and 30 cases for normal as control group.Scan MPV,left and right branch of portal vein, SMV,SV and measure diameters of these vasculars on MIP by using 16-slice spiral CT.Measure size of liver,spleen and left lateral lobe live by using measure software(support by CT).Calculate size ratio of liver and spleen,left lateral lobe live and liver.Observe the opening collateral circulation.Control study and statistical analyze these indicators and MELD score.Results 1,diameters of normal portal vein:MPV(1.15±0.11) cm,left branch of portal vein(0.82±0.14) cm,right branch(0.94±0.13) cm,SV(0.78±0.11) cm,SMV(1.08±0.20) cm.diameters of portal vein in liver cirrhosis were Significantly broader than the normal group, were MVP(1.36±0.22) cm,left branch of portal vein(0.93±0.20) cm,right branch(1.03±0.21) cm,SV(0.91±0.25) cm,SMV(1.22±0.17) cm。2,MPV and SMV increasing in MELD<15 group and MELD>25 group,were(1.32±0.17) cm,(1.18±0.17) cm;(1.34±0.24) cm,(1.21±0.15) cm;(1.52±0.22) cm,(1.34±0.16) cm.3 groups all had significantly differences to the normal group.MELD >25 group and MELD scored 15 to 25 group,MELD≤15 had statistical differences. Diameters of left branch of portal vein MELD higher than 25 group were (0.99±0.27) cm,had statistical differences to normal group,the other two groups had no difference to normal group,but the diameters,higher than the normal group,were(0.91±0.19) cm;(0.92±0.17) cm.Diameter of right branch of portal vein on patients group were(1.03±0.21)cm,(1.06±0.21) cm,(1.02±0.20) cm.15-25 group had statistical difference to normal group,SV group scored 15 to 25 and higher than 25 had statistical difference to normal group,P<0.05,the other two groups had no difference to normal group.3,75 cases in liver cirrhosis showed collateral circulation opening up.The rate of collateral circulation opening up in MELD scored higher than 25 reached 86%.3,Size of spleen was positively correlated to MPV,SV,SMV.Related coefficients were 0.415,0.496,0.322.4,MPV,SV,SMV and spleen size were positively correlated,correlation coefficients were0.415,0.496,0.322。5,The size of normal liver was(1211.87±360.59) cm~3,the size of patient groups were(1066.78±269.91 ) cm~3,(1019.05285.55 ) cm~3,(844.44±164.80 ) cm~3.The three patient groups had statistical differences to the normal group.MELD>25 group and MELD≤15, MELD between 15 and 25 group had statistical difference.The size of normal spleen was(171.48±79.31) cm~3,the size was increasing in group of MELD≤15 to MELD>25.The three patient groups had statistical differences to the normal group.MELD>25 group and MELD≤15,MELD between 15 and 25 group had statistical difference;size ratio of liver and spleen on normal group was(8.07±3.06),reducing on condition,were(2.91±1.98),(2.76±2.08),(1.49±0.81),three groups had significantly differences,the group of MELD between 15-25,MELD >25 and MELD≤15 group had in-group differences.6,Size of left lateral lobe liver scored equal or less than 15 by MELD increased significantly and had statistical differences to the other 3 groups.The ratio,size of left lateral lobe live and liver,was significantly higher than the normal group.Conclusion 16-slice CT played an important role in the evaluation of diagnosis of liver cirrhosis and liver function by measuring the diameters of portal system,sizes of entire liver,left lateral lobe of liver and spleen, and observing the extend of collateral circulation opening up.
