Objective: Gestational trophoblastic diseases (GTD), included hydatidiform mole, invasive mole, choriocarcinoma and placental site trophoblastic tumor (PSTT). Invasive mole, Choriocarcinoma and Placental site trophoblastic tumor (PSTT) are collectively refer to as gestational trophoblastic tumor. Hydatidiform mole is a benign lesion. But it has the trend of malignant transformation. About 20 percent complete hydatidiform mole and 4 percent partial hydatidiform mole have the potential to develop into gestational trophoblastic neoplasia (GTN). Its mechanism is unknown, may be related to immune factors. Immunosuppressive acidic protein (IAP) is an immunological indicator as a reflection of immune status. IAP is produced by macrophages and granulocytes et al. When the body by pathogrnic factor infringed upon, IAP were significantly higher. In recently years, studies suggested that IAP can be used as potent biological marker in patient with malignant tumors. The level of serum IAP can be used as assistant diagnosis, judging the malignant degree and predicting the prognosis. This is the first report analyzing the significance of serum IAP level in gestational trophoblastic disease. Our research through enzyme-linked immunosorbent assay determin the expression of IAP in hydatidiform mole,invasive mole and choriocarcinoma. The objective are to explore immune status of the patients with the gestational trophoblastic disease and the mechanism of malignant transformation of hydatidiform mole. provide A theoretical basis was provided for gestational trophoblastic tumor immunotherapy.Methods:1 The subjects in this study were women of treatment for the gestational trophoblastic disease between march 2006 and June 2008 in the department of Gynecology of the third Hospital of Hebei Medical University. 40 cases with gestational trophoblastic disease(Experimental group) and 8 health women of childbearing age (control group) were evaluated. Of the 40 cases in experimental group,12 cases were hydatidiform mole (4 cases of deteriorative hydatidiform mole), 24 cases invasive mole, 4 cases choriocarcinoma. Stages of the disease and diagnosis were classified according to the Obstetrics and Gynecology seventh.2 Materials 4ml venous blood were collected and placed at room temperature for 2 hours and separated at 3000 rev / min for 20 minutes. Sera were stored at -20℃refrigerator until assay, The level of IAP was detected by enzyme-linked immuno- sorbent assay.3 Statistical analysis The level of IAP were expressed as mean±SD. All statistical calculations were carried out with SPSS version 13.0. The significance of differences between groups was calculated by analysis of variance (ANOVA) and the two-two comparison among the means was done by LSD-t method. P<0.05 was considered statistically significant.Results: 1 The level of serum IAP is express in each group (health women, hydatidiform mole, invasive mole and choriocarcinoma).2 The level of serum IAP in patients with hydatidiform mole, invasive mole and choriocarcinoma is significantly higher than control group. The difference between them was statically significant (P<0.05).3 The level of serum IAP in patients with deteriorative hydatidiform mole is significantly higher than non-deteriorative hydatidiform mole. The difference between them was statically significant (P<0.05).4 The level of serum IAP in the mole, invasive mole, choriocarcinoma was not statically significant, either (P>0.05).Conclusion: 1 The level of serum IAP is detected in healthy women of childbearing age, hydatidiform mole, invasive mole and choriocarcinoma.2 The level of serum IAP in hydatidiform mole, invasive mole and choriocarcinoma is higher than that of the health women. IAP may be take part in the course of immune injury and provide a theoretical foundation with immune therapy study for gestational trophoblastic disease patients.3 The level of serum IAP in patients with deteriorative hydatidiform mole is significantly higher than non-deteriorative hydatidiform mole. The level of serum IAP can be used as a marker for malignant transformation of hydatidiform mole and had clinical value for predicting prognosis of hydatidiform mole.
