Objective:To investigate the expression of NF-κBp65 and to evaluate its clinical significance in the occurrence and development of vulvar epithelial nonneoplastic disorders and vulvar squamous cell carcinoma(VSCC).Methods:Using immunohistochemical technique(EnVision),NF-κBp65 was detected in 265 cases of vulvar epithelial nonneoplastic disorders(squamous hyperplasia of vulva 123 cases,lichen sclerosus 95 cases and squamous hyperplasia accompanied with lichen sclerosus 47 cases),13 cases of vulvar intraepithelial neoplasia and 33 cases of vulva squamous cell carcinoma.All cases were chosen from outpatient and inpatient of Women's Hospital School of Medicine Zhejiang University in 1997-2008.Thirty cases of normal control were taken from the normal skin beside the lesion skin.Results:Except 49.5%cases of lichen sclerosus(LS) expressed NF-κBp65, positive expression rate of NF-κBp65 in other groups were 100%.In normal vulva skin,NF-κBp65 expressed in all epidermis cells cytoplasm and the basal layer slightly higher.NF-κBp65 mainly expressed in basal cells cytoplasm in LS and in both basal cells and prickle cells cytoplasm in squamous hyperplasia(SH) and vulvar intraepithelial neoplasia(VIN).In SCC,there appeared strong positive signals in nests.The rates of positive signal of nucleus in SH,VIN,LS,SCC and mixed type were 47.2%,53.8%,7.4%> 27.3%and 6.4%respectively.All groups displayed positive signals in fibroblast and vascular endothelial cells cytoplasm in dermis.In VSCC,vascular endothelial cells nucleus were stained too.The expression of NF-κBp65 in LS was lowest among six groups and had significant difference with other groups(all P=0.000);There were no significant difference between the expression of NF-κBp65 in VIN and VSCC(P=0.351) but they were higher than other four groups.NF-κBp65 expressed higher in SH than normal skin,LS and Mixed groups(P=0.002,P=0.000,P=0.043).The expression of NF-κBp65 in mixed group was higher than LS(P=0.000) but had no significant difference with normal skin(P=0.105).There were no significant difference of the NF-κBp65s expression among three groups of VSCC((P=0.759,P=0.891,P=0.694).Conclusion:The results suggested that NF-κB p65 may play an important role in the occurrence and development of vulvar epithelial nonneoplastic disorders.
