The Study On Anorectal Motility In Patients With Functional Constipation

Objective:To study the anorectal motility in patients with functional constipation. Methods:The anorectal manometry was done in 30 patients with functional constipation (FC) from March 2007 to December 2008 and 30 normal controls.According to RomeⅢcriteria,30 cases with FC were classified into three subtypes:10 cases with slow transit constipation(STC),11 cases with dyssynergia(F3a) and 9 cases with deficiency of propulsion(F3b).The length of anal,rectal sensation threshold,rectal sensation of defecation threshold,maxium tolerable volume threshold,rectoanal inhibitory reflex (RAIR),anal resting pressure,anal squeeze pressure,coughing anal pressure,rectal resting pressure,rectal squeeze pressure,rectal defecation pressure and anal defecation relaxation pressure were measured by anorectal function detector of polygraf. Results:The length of anal had no difference between patients with FC and the normal controls.The rectal sensation threshold and rectal sensation of defecation threshold in patients with FC were significantly higher than in normal controls(P<0.05).No difference of maxium tolerable volume threshold and rectoanal inhibitory reflex(RAIR) between patients and normal controls.The anal resting pressure was lower in patients with FC(P<0.05 ),the anal squeeze pressure,coughing anal pressure,rectal resting pressure,rectal squeeze pressure and rectal defecation pressure were not different from normal controls.But the rectal defecation pressure in patients with F3b was lower than in normal controls(P<0.05 ).The anal defecation relaxation pressure was significantly lower in normal controls and in patients with F3b(P<0.05 ),moderate lower in patients with STC(P>0.05),and obviously higher in patients with F3a(P<0.05 ) than anal resting pressure.Conclusion:These show that changes of anorectal dynamics and sensation in patients with functional constipation,and different subtypes of functional constipation dose not have the same manifestation with anorectal manometry.The anorectal manometry is significant in typing and directing treatment for patients with FC.
