Objective:To detect serum level variation of tumor necrosis factor- a (Tumor Necrosis Factor- a, TNF- a), platelet-activating factor (Platelet Activating Factor, PAF) and interleukin -10 (Inter leukin-10, IL-10) in patients with acute lung injury. Observe the changes trends in patients with ALI and to explore its clinical significance。Methods: To select 30 cases of patients with acute lung injury By the Chinese Medical Association of Respiratory Diseases credits will be established diagnostic criteria for ALI, as a lung injury group, and select 30 cases of healthy control group, to take peripheral venous blood 5ml after 1d, 3d, 5d, 7d in lung injury group admitted to hospital admission, to take peripheral venous blood 5ml from the control group, using ELISA method of detection of 30 cases of healthy persons and patients with acute lung injury in serum TNF- a, PAF, IL-10 concentrations at various time periods and changes in correlation analysis.Results:1,Lung injury group, TNF-a, PAF, IL-10 concentrations were 155.80±64.12 pg / ml, 12698.84±3721.98 mU / ml, 138.89±57.47 pg / ml, control group, TNF-a, PAF, IL-10 concentrations were 74.6218±14.83 pg / ml, 8500.23±1376.62mU/ml, 54.7±13.57 pg / ml. Compared with healthy control group, lung injury group, TNF-a, PAF, IL-10 concentrations were significantly elevated (P <0.01)2,Lung injury group, TNF- a section 1d, 3d, 5d, 7d, respectively, for the concentration of 222.89±56.11 pg / ml, 177.01±46.11pg/ml, 128.99±36.05pg/ml, 94.31±22.8pg/ml. TNF-ɑwas significantly increased at 1d after injury and reached a peak after a downward trend, close to a normal level after one week, compared with healthy control group, day 7 TNF-a concentration was still significantly elevated (P <0.01) .3,Lung injury group, PAF section 1d, 3d, 5d, 7d, respectively, for the concentration of 11885.41±3143.31 mU / ml, 13429.51±3403.17mU/ml, 14160.82±4286.27mU/ml, 11319.60±3405.24mU/ml,PAF began to increase at 1d after trauma, to 3 ~ 5d peak, compared with the control group, the 7th day there is decreasing.4,Lung injury group, TL-10 section 1d, 3d, 5d, 7d, respectively, for the concentration of 209.01±29.88pg/ml,162.87±29.02pg/ml,110.99±22.36pg/ml,72.71±17.38pg/ml, IL-10 was significantly increased at 1d after injury and reached a peak after a downward trend, close to normal levels after one week. Compared with the control group, the first 7 days, the concentration of TL-10 Change is still elevated, (P <0.01).Conclusion:1,TNF-a, PAF and IL-10 serum expression in acute lung injury has obvious significance.2,This experiment suggests that TNF-a, PAF and IL-10 detection in lung injury diagnosis, treatment and prognosis is expected to become a good predictor.3,At present, many animal experiments found that TNF-a antibody, PAF antibodies, pentoxifylline and other drugs of pathogenesis can be reduced lung injury, but in clinical application was blank, for the future should further develop their applications in clinical research, that is, for the regulation of the inflammatory process, it is excepted that application of the drug of the mechanism of disease for ALI / ARDS treatment can bring a historic breakthrough.
