Objective To investigate the effect of Maxillofacial surgical patients' Plasma concentration of beta-amyloid1-40 protein[Aβ(1-40)] and the products of nitric oxide(NOx) on postoperative cognitive dysfunction(POCD) of elderly patients undergoing general anesthesia. Methods Sixty ASAⅠ~Ⅱsurgical patients aged 30 to 85 years old, which were assigned the time limit operation of maxillofacial malignant tumors or fractures. 60 patients were divided into elder group (30 cases, aged 60~85 years) and young and middle-aged group (30 cases, aged 30~55 years). At preoperative 1d and 1~7d after operation all patients brought into the experiment were carried out neuropsychological testing by mini-mental status examination(MMSE). After each neuropsychological testing sampled intravenous blood and checked the concentrations of blood plasma Aβ(1-40) and NOx. Results Elder group has 11 POCD cases and 2 patients in young and middle-aged group have gotten POCD. In two groups,compared with the preoperative concentrations of plasma NOx and Aβ(1-40), patients who were POCD at different time points after operation have a higher the concentrations and the elder group's the concentrations increased significantly more durable than the young and middle-aged group's (P <0.05).POCD patients compared with non-POCD patients, POCD patients' concentrations of the plasma Aβ(1-40) and NOx increased significantly and lasting(P <0.05). Conclusion After surgery elderly patients more susceptible to POCD than young and middle-aged patients. After operation patients'the concentration of plasma Aβ(1-40) and NOx remained at higher levels,which may be one of the factors related to POCD for elderly patients. Can the concentration of Plasma Aβ(1-40) and NOx be regarded as the early-warning indicators of POCD for the elderly patients after operation? That must depend on the results of further multi-center, larger study sample clinical trials to evaluate.
