Objective:To observe the effect of the medicined diet on intestinal mucosa pathomorphology,endotoxin level,intestinal bacterial translocation rate,the change of cecum mucosal microfor and the level of sIgA.Understanding the effect of preventing and curing of gut derived infection by medicined diet, to probe a new way to prevent and cure the gut derived infection after burn.Methods:To design a kind of medicined diet, According to the making methods of medicined diet to decoct traditional Chinese drug into the medicinal broth. 100 healthy Wistar rats with 50% female and 50% male wighting 200±20 g were randomly divided into four groups:burned with lavaging medicined diet group (medicined diet group,n=30),burned with lavaging bouillon group (bouillon group,n=30),burned with convention feeding group(convention group,n=30) and normal control group(control group,n=10).After depilated,anesthetized,the rats were burned on the back about 30%TBSAⅢdegree through pathological verificated.The rats of the control group were fake burned in 37℃.Except given the convention food,the rats of the medicined diet,bouillon and convention group were given medicined diet,bouillon and NS seperately after 2 h post-burn. The wound of the rats were besmeared iodophor,2 times everyday. The rats of control group were killed for sampling just after the begining of experiment.At 1,3,7 PBD,10 rats of the the medicined diet,bouillon and convention group were also killed for sampling in the condition of asepsis to detect(:1)intestinal mucosa Pathomorphology observation (2) the endotoxin level (3) the intestinal bacteria translocation rate (4) cecum mucosal microfor(5)the level of sIgA.All the experimental datas were analyzed by SPSS 15.0.Results:1. Intestinal mucosa pathomorphology observation:compared with the control group, different pathomorphological changed in intestinal mucosa of medicined diet,bouillon and convention group after scalded. Intestinal mucosa obvious atrophy, the epithelial cells of mucosa necrosis,off, goblet cells increased, heavy inflammation,submucosa thinning and loosen,muscularis thinning or fracture. Medicined diet group rat basically recoverd to nomal at the post burn 7th day,but intestinal mucosa of bouillon and convention group rat didn't completely recoverd.2. The intestinal bacterial translocation rate and the endotoxin level: Compared with normal group, the intestinal bacterial translocation rate and the endotoxin level of medicated diet group,bouillon group and convention group were increased significantly at every phase point after scald(P<0.01 or P<0.05);Compared with bouillon,convention group,the bacterial translocation rate and the endotoxin level of medicined diet group were decreased (P<0.05 or P<0.01).3. Caecum membrane flora Change:Compared with normal control group,the bifidobacteria of medicined diet,bouillon and convention group was decreased while the yeast and E. coli was increased at every phase point after scald(P<0.05 or P<0.01);Compared with bouillon,convention group, the bifidobacteria of medicined diet group increased while the yeast and E. coli were reduced apparently at every phase point after scald ( P <0.01 or P <0.05).4. The lever of sIgA: Compared with the control group,the lever of sIgA of the medicined diet,bouillon and convention group was apparently decreased at every phase point after scald(P<0.01or P<0.05),have statistical significance; Compared with bouillon and convention group,the lever of sIgA of medicined diet group was increased apparently at every phase point after scald(P<0.01or P<0.05).Conclusions:The barrier function of intestinal mucosa was weakened after severely burned, caused by bacterial and endotoxin translocation. Application of traditional Chinese medicine diet treatment, promoted the restoration of intestinal barrier function, reduced bacterial and endotoxin translocation, adjusted the balance of intestinal micro-ecological, increased intestinal local and systemic immune function. So, achieved the purpose of preventing and treating the gut derived infection.
