| Objective : to know the effect of the operations especially the radical operations of craniopharyngioma and to assess the outcome of endocrine functions disorder after craniopharyngioma resection .Methods A total of 121patients (71 men and 50 women) with craniopharyngioma were microsurgically removed by Pro.Yuan XR(MD) himself from October 1991 to October 2009.A retrospective review was performed. The general information, preoperative symptoms, signs, tumor size, cystic formation, endocrine function, etc were recorded. An average of 35 months' follow-up was performed in 82 patients, including 72 patients received total tumor removal, and 10 underwent subtotal or partial removal. And the endocrine function was assessed by spss13.0.Results :the average tumor size is 42.3mm . Total removal of the tumors are achieved in 98(81.0%). Total removal of the tumors were achieved in 82 (92.1%, 82/89) patients after the year 2002. The pituitary stalk was anatomically preserved in 26 (21.5%) patients. Two (1.7%) patients died during perioperative period. During the follow-up, 6 (8.3%) patients experienced tumor recurrence after total tumor removal.70cases 82 cases of patients score 80 points or more in Karnofsky criterion. the characteristics of hormones changes : 1,Craniopharyngioma patients with abnormal hormone occupy low rate preoperatively. 2,Five hormones all decrease after surgery . 3,the patients with pituitary stalk preserved have a better prognosisConclusion 1. effect of this group of 121 cases craniopharyngioma is significant after surgery, locate in the ranks of the international advanced level. 2. postoperative endocrine function is related to preoperative endocrine function and pituitary stalk preservation, not related to tumor size, calcified, or cystic... |
