Objective:to analyze the Prevalent characteristics of rabies from 1998 to 2007 in Shaoyang City,to explore the derterminants of vaccination post-exposure rabies and molecular epidemiology of rabies virus in Shaoyang City,tnen provide scientific basis for rabies prevention.Methods:Using Excel 2003 and statistical software SPSS13.0 analyze the Collected data.Using ratio and composition ratio do statistical description of count data.Usingχ~2 test and non-conditional logistic regression analysis do Univariate analysis and Multivariate analysis of post-exposure rabies vaccination respectively.Using BLAST in GENEBANK analyze the nucleotide sequence homology to determine the genotype of rabies virus,using MegAlign of the DNAStar software package study N gene sequence homology of the internal strains and between the studying strains and Vaccine strains,and Using NJ methods(Neighbor-Joining,adjacent to the Law)build phylogenetic trees.Results:Over the past ten years(from 1998 to 2007),a total of 380 human rabies cases were reported in Shaoyang City,with an annual average incidence rate at 0.53 per 100 thousand population.The epidemic situation had a Inverted"V" shape distribution trend - former ascend and later descent.The regional distribution:the city's 12 counties(cities) had reported cases,but the majority of cases were distributed in Shaoyang County,the Xing shao county,Dongkou county,Wugang city,Shaodong county these five areas;Most of the cases were occurred from July to November,while fewer occurred from March to April and December. The most susceptible group were farmers,secondly were students,The age group from 50 to 55 years took the most cases,The gender proportion of cases is 1.75:1(Men:women).Univariate analysis showed that Exposed parts(χ~2=33.245,P=0.000),wounds treatment way(χ~2=50.397, P=0.000),wounds disinfection(χ~2=5.961,P=0.015),wounds suture (χ~2=9.079,P=0.003),pre-exposure immunization vaccination (χ~2=12.830,P=0.000) were associated with post-exposure rabies vaccination;Multivariate non-conditional logistic regression analysis showed Medical institutions treatment of wounds treatment way (OR=10.579,95%CI:2.564~43.652),wounds disinfection (OR=10.7882,95%CI:1.016~114.505) were associated with post-exposure rabies vaccination,the 7-positive genotyping products obtained by the nested RT-PCR detection method were type 1 gene; seven rabies virus N gene fragment(704~1423bp) Nucleotide sequence homology ranged from 97.4%to 100%(median 98.1%),Nucleotide sequence differences in the range of 0.0~2.7%(median 2.0%),amino acid sequence homology ranged from 99.6%to 100.0%(median 99.6%), amino acid sequence differences in the range of 0.0~0.4%(median 0.4%);the scope of the nucleotide homology between 7 strains of shaoyang and the representatives of four vaccine strains was 86.7% 90.3%(median 89.2%),differences in the range of 10.5%~15.2% (median 12.0%),amino acid homology ranged from 96.7%to 99.6% (median 99.2%),differences in the range of 0.4%~3.4%(median 0.8%).Conclusion:The epidemic trend of rabies from 1998 to 2007 in Sha -oyang City was a inverted V-shape.The geographical scope of rabies was spreading.Cases Ranked the top three werec Shaoyang county,Xing shao County and Dongkou county.Cases of men were more than women'.The age group from 50 to 55 years took the most cases,the most cases' occupation is farmers.The incidence of rabies showed a clear Summer and autumn seasonal.Crowd after exposure had a low rate of treatment and vaccination.Wounds treatment way and wounds disinfectio -n.The two variables were associated with post-exposure rabies vaccination.Seven rabies virus in Shaoyang City were type 1 gene.The nucleotide and amino acid homology of strains is basically similar.The N protein gene structure,which was relatively stable,was the synonymous mutation gene mutation.Homology of the N gene between studying strains and domestic vaccine,the studying strains had the highest homology witn the CTN strain.The phylogenetic analysis showed that all strains could be broadly divided into two groups.The Studying strains have a closer relationship with the vaccine strains and the vast majority of strains over the years.
