Objective:To evaluate the incidence rates of postoperative complications of the laparoscopic resection for colorectal caner, comparing to the open surgery. Methods:Articles of randomized controlled trials(RCTs) concerning the incidence rates of postoperative complications of the laparoscopic resection and open surgery for colorectal caner which were published in the last decade were retrieved through Pubmed, and correlated indexes were extracted for meta-analysis.Results:Six articles were selected and evaluated. The total number of the cases was 3060(male 1599 and female 1461).And the number of the cases for laparoscopic resection was 1563 and for open surgery 1497.TNM staging:stageâ… 636,stageâ…¢1207.Results originated from the software RevMan 4.2 showed that the basic characteristics of patients in laparoscopic resection group were similar to those in open surgery group(age:WMD=0.30,95%CI:0.49-1.95,P=0.45;male proportion: OR=0.90,95%CI:0.78-1.04,P=0.16;staging I:OR=0.96,95%CI:0.81-1.15,P=0.68; stagingâ…¢:OR=0.91,95%CI:0.78-1.06,P=0.24) The incidence rates of the postoperative complications of the two groups were:(1) the incidence rate of bowel obstruction in the laparoscopic group was lower than the open surgery group (OR=0.57,95%CI:0.38-0.85,P=0.006).(2) the incidence rate of cardiac complications in the laparoscopic group was lower than the open surgery group (OR=0.49,95%CI:0.25-0.96,P=0.04).(3) there were no significant differences upon other complications between the two groups(intraabdominal hemorrhage:OR=1.31, 95%CI:0.76-2.26,P=0.33;fistula of anastomotic stoma:OR=1.00,95%CI:0.65-1.55, P=1.00;incision infection:OR=0.83,95%CI:0.66-1.03,P=0.09;urinary retention: OR=1.23,95%CI:0.62-2.42,P=0.55;infection of the urinary system:OR=0.92, 95%CI:0.59-1.44,P=0.71;infection of the respiratory system:OR=0.91,95%CI: 0.64-1.31,P=0.62).Conclusions:The results of meta-analysis show that the laparoscopic resection for colorectal cancer can reduce the incidence rates of bowel obstruction and cardiac complications and there are no significant differences of other complications between the two groups.
