| Objective: To investigate the outcomes of Fulkerson osteotemy and lateral patellar retinaculum release with patellofemoral ligament reconstru- ction using the semitendinosus tendon to treat the recurrent dislocation of the patella. To afford an effective method of recurrent patellar dislocation surgery treatment and discuss its indications.Methods: 17 patients (17 knees) who suffered from the disease of recurrent patellar dislocation were diagnosed with symptom, medical record,medical examation and radiographic methods. 12 patients (12 knees) were treated by modified Fulkerson tibia tubercle anteromedial transfer (Type I).5 patients (5 knees) were treated by patellar lateral retinaculum release with medial patellofemoral ligament reconstruction (Type II).15 patients (15 knees) were followed up. The average follow-up period of the 10 patients who were admitted with operation Type I is 49.7 months (26—78months), the average period of the other 5 patients who were admitted with operation Type II is 7 months (2—14months).Patients were assessed before and after surgery with the kujala patellofemal score measure and compared the indexes which describle the apposition of patellofemoral joint including CA, LPA, LS by Ficat's axial view radiographs of the patella.Result: The kujala patellofemoral score was 55±11.05 and 53.8±8.56 preoperatively. The kujala patellofemal score was 78±4.4 and 66.2±3.63 postoperatively. There were statistical differences (P<0.01) between preo- peration and postoperation. It was showed that the function of patellofe- moral joint was changed significantly. The comparison of indexes of Ficat's axial view radiographs displayed CA, LPA, LS were all changed signify- cantly (P<0.01).Conclusion:Two types of operation could rectify the maltracking of patellofemoral joint thoroughtly,relieve patellofemoral pain and improve the function of patellofemoral joint. |
