Objectives: To explore the morphology change of the coronary venous system in hypertensive patients by the multi-slice spiral CT (MSCT).Methods: Randomly selected 100 cases of hypertensive patients who accept multi-row CT coronary angiography from March 2008 to October, including 47 cases of female, average age 62.3±9.7 (35-78) years old, and the exclusion of congenital heart disease, valvular heart disease, primary cardiomyopathy, primary and secondary pulmonary hypertension and arrhythmia. We studied by contrast enhanced MSCT which retrospective ECG-reconstructions. The morphologic, length, ostium, number, diameter and morphology of hypertension patients'coronary sinus and its branch were measured from both 3-dimersional volume rendered images and 2-dimersional cured planar images, and compared with the results in healthy adults 50 cases who accept multi-row CT coronary angiography. The main purpose was to observe the morphology change of the coronary venous system. A probability value p<0.05 was considered statistically significant, p<0.01 was considered quite statistically significant.Results: 1,The coronary sinus morphology of hypertensive patients and healthy adults: In hypertensive patients the tubular coronary sinus was 75 cases (75%), and bell-shaped was 17 cases (17%), and 8 cases (8%)of coronary sinus dysplasia. In healthy adults the tubular coronary sinus was 38 cases (76%), and bell-shaped was 10 cases (20%), and 2 cases (4%)of coronary sinus dysplasia.2,The diameter of coronary sinus and its branches in hypertension patients:The average diameter of the coronary sinus orifice was 10.5±3.2(4.2-20.8)mm. The total length of the coronary sinus was 35.6±9.0(13-61)mm. The coronary sinus average diameter in the middle cardiac vein was 9.8±2.2(5.5-15.8)mm. The coronary sinus average diameter in the left atrial oblique vein was 8.6±1.9(5.2-14.4)mm. The middle cardiac vein average diameter was 5.2±1.4(2.2-10.5)mm. The diameter of coronary sinus and its branches in healthy adults: The average diameter of the coronary sinus orifice was 10.6±3.5(4.6-17.9)mm. The total length of the coronary sinus was 34.8±7.5(13-61)mm; The coronary sinus average diameter in the middle cardiac vein was 8.9±2.2(3.8-11.8)mm; The coronary sinus average diameter in the left atrial oblique vein was 7.9±1.4(5.2-10.4)mm; The middle cardiac vein average diameter was 5.1±1.2(3.1-7.2)mm. 3,The main type of coronary sinus in hypertensive patients and in healthy adults is balanced-type dominated. 4,The difference in the hypertensive patients and healthy adults: There were significant differences in the coronary sinus average diameter in the middle cardiac vein and in the left atrial oblique vein.Conclusion: Hypertension on coronary sinus orifice diameter and morphological variation diameter and the length of the coronary sinus was no effect, but the coronary sinus average diameter in the middle cardiac vein and in the left atrial oblique vein magnified.
