Background At present, a more satisfactory treatment of bone defects is autogenous bone graft, but there are many faults , such as limited sources, the impact of reactive functional , pain and infection, so clinical application is limited . Find a substitute of autogenous bone material has become the Orthopedic Research hotly. Bone cement has become the hope of the fracture repair materials because of its unique advantages. However, recent studies show that bone cement have less osteoinduction and slow degradation in the body , so the effect of the clinical application separately is not satisfactory. Therefore, people have modified them, hoping to resreach a new materials that can overcom all shortcomings above-mentioned.Abstract Objective To investigate the ability of DBM,ABC ,and BPM in filling and repairing bone defect and the best component proportion of the composition . Design, time and place Randomized controlled animal experiments, in May 2008 ~ September 2008 in the experimental animal of Chongqing Medical Center.Material Acrylic Bone Cementⅲ(containing reagent), Decalcified Bone Matrix (including recombinant human bone morphogenetic protein 2, recombinant human bone morphogenetic protein 4, insulin-like growth factor 1, transforming growth factor-β1).Method The animal models of bilateral femur bone defects and fillers were established in the New Zealand White rabbits. DBM and ABC were mixed at different weight proportions. The complex material was examined by biomechanics ,scan electron microscopse and was implanted on the bone defect, the ABC was used as blank control group .Main outcome measures Gross observation , histopathological and X-ray test were used at 4, 8, 12 weeks after operation respectively to observe the repaired and filled status of bone defect.Result 1. DBM composite quality of the scope of 3:7 ~ 6:4, the composite materials that exist in more cracks more than 100μm, when the DBM is less than 3:7 ratio, the material inside the aperture and the majority of material is similar to ABC, mass ratio greater than 6:4 when the DBM and the ABC can not effectively together solidification. 2When the mass ratio is 2:8 ~ 6:4 which matrix particles of demineralized bone and acrylic bone cement ,the compressive ultimate strength of composite materials is less than pure acrylic bone cement (P<0.05).3.After observing the result above at different time, we found the new bone to form on the bone of defected and filled. On the 12th week, when the proportion of DBM was from 0.2 to 0.6,the osseointergration percentages were (87.35±3.62)%,(89.67±4.38)%,(92.83±3.97)%,(90.12±4.01)%and(88.36±3.65)%.A significant difference ( P < 0. 05) was found in the statistical comparison of the different groups with regard to the composition's ultimate osseointergration percentages.Conclusion Along with the increasing proportion of DBM, the porosity of compositions is ascended, contrarily the compressive strength of compositions is descended accordingly. .when the proportion of DBM was 0.4,it has the best osseointergration percentages. Compositions with DBM proportion of 0.4 can be best used in repairing cancellous bone's gap where bears lower had. |
