The Effect Of Coagulation Function On The Volume Of Blood Loss, The Neurological Deficit Scores And The Prognosis In The Patients With Acute Spontaneous Intracerebral Hemorrhage

Objective:To explore the alteration law and clinical significances of these plasma sensitivity parameters (fibrinogen,the conditions of platelets,D-dimer and Antithrombin III activity) in thirty patients with acute spontaneous intracerebral hemorrhage and to evaluate the relationship between them and the size of intracerebral hematoma, the severity of brain injury and patients'outcomes. Methods:We detected the levels of Fbg,the conditions of platelets,DD and AT-lll activity by chromogenic analysis method and latex-enhanced immunoturbidimetric test within 48 hours after admission,the neurological deficit scores in the admission and the prognosis(modified Rankin scale scores) after three months of injury in thirty acute spontaneous intracerebral hemorrhage patients. Their levels were compared with control group.And we evaluate the relationship between them and the size of intracerebral hematoma, the neurological deficit scores and patients'outcomes.Results:①The levels of Fbg,At-III activity,the conditions of platelets and DD in acute spontaneous intracerebral hemorrhage group were differet and the differences were signifieance(P<0.01) compared with control group.②There was a negative correlation between the levels of At-III activity and the size of intracerebral hematoma, the neurological deficit scores and the patients'outcomes. (P<0.05).③There was a negative correlation between the levels of Fbg and the size of intracerebral hematoma and the patients'outcomes. (P<0.05).④There was a negative correlation between the levels of DD and the size of intracerebral hematoma, the neurological deficit scores and the patients'outcomes.(P<0.05).⑤There was no correlation between the levels of the conditions of platelets and the size of intracerebral hematoma, the neurological deficit scores and the patients'outcomes. (P<0.05).Conclusion: Acute spontaneous intracerebral hemorrhage patients present a state of hypercoagulability and secondary hyperfibrinolysis at early stage. There was a positive correlation between the levels of Fbg and DD and the size of intracerebral hematoma and patients'outcomes in the acute spontaneous intracerebral hemorrhage group,and There was a negative correlation between the levels of At-III activity and the size of intracerebral hematoma, the severity of brain injury and patients'outcomes. We can estimate the severity of intracerebral hemorrhage of the patients by early detecting the levels of Fbg,DD and AT-lll activity, the clinical value is remarkable.
