Oral lichen planus(OLP) is a chronic inflammatory disease of oral mucosa that mediated by T lymphocyte.OLP is characterized by an extensive subepithelial infiltrate of T cells,together with degeneration of basal cells and break of basement mambrane.The etiology and pathogenesis are still obscure.Much effort has been spent on OLP therapy,but the outcome is still far from satisfaction.There are extensive data to suggest that immunifaction play an important role in invasion and development of OLP.Previous reports revealed that invasion of OLP is a process of immunologic injury by T cell,concerned with autoimmune function of body.The infiltrate of OLP is maily T lymphocyte,what regular change does it's quantity,distribution,proportion of it's subgroup and production have? How to stop or to deteriorate these changes? These are focal points of this research.Objective:To explore the role of T cell in morbility of oral lichen planus,we assess the distribution of CD4~+T lymphocytes,CD8~+T lymphocytes,Th1cells,Th2cells by immunohistochemistry and the expression of IFN-γ,IL-10 by RT-PCR and real time PCR respectively.Methods:1.OLP(n=45) and normal control(n=16) tissues were defined clinically and pathologically.Eight serial sections were cut for HE staining,immunohistochemical staining and negative control(3 sections) respectively.We observed the expression of of CCR3,CCR5,CD4 and CD8 in different tissues at the levels of protein using IH.2.OLP(n=11) and normal control(n=5) tissues were defined clinically and pathologically.To detect the expression of IFNγ,IL-10 using PCR technique and real-time PCR.Results:1.Compared to normal oral mucosa,counts of CD4+T lymphocyte,CD8+T lymphocyte,Th1 cell and Th2 cell is increased.And difference exist during their distribution.2.The expression of IFNγmRNA is higher while the expression of IL-10mRNA is lower in OLP.Values of P<0.05 were considered statistically significant.Conclusion:1.The augmentation and distribution difference of CD4+T lymphocyte,CD8+T lymphocyte,Th1 cell and Th2 cell suggest various kinds of T lymphocytes play different roles in invasion and development of OLP.2.The variation of IFNγ,IL-10mRNA in OLP suggests they produce a marked effect in OLP development and progression.To consider the anti-inflammatory effect of IL-10 and the pro-inflammatory effect of IFNγ,it suggests inflammatory reaction play an important role in OLP development and progression.
