| Objective To investigate the changes of plasma visfatin levels and the relationship between plasma visfatin levels and glycose and lipid metabolism in gestational diabetes mellitus.Methods Women with primary diagnosis of gestational diabetes mellitus were choosed between 28 weeks and 34 weeks in GuangDong Maternal and Children's Hospital.Among the total,GDM group 38 cases,GIGT group 27 cases and control group 35 cases with normal glucose and matching the experiment group of age,weeks,weight. All the cases were picked forearm vein blood after limosis,were detected plasma visfatin by ELISE, serum glucose,insulin , triglyceride, total cholesterol, low density lipoprotein, high density lipoprotein, compute homa-insulin resistance index(HOMA-IR)and homeostasisβ-cell function index(HBCI).Measure high,weight, inquire progestation weight,and compute body mass index(BMI).To analyze the plasma visfatin level in different glucose tolerance and relationship of plasma visfatin level with FPG, FIns, TG, TC, HDL, LDL, HOMA-IR, HBCI,body weight adding during gestational period, progestation BMI. Picked forearm vein blood after limosis once more during 37~40weeks,detected plasma visfatin by ELISE,to investigate the changing of the plasma visfatin level in different weeks. Results1.Between 28 and 34 weeks,fasting plasma visfatin levels in the GDM group and the GIGT group are significantly reduced compared with controls(14.13±6.86 ng/ml,16.26±6.37 ng/ml vs 22.74±5.18 ng/ml,P<0.01),and the difference of the level of plasma visfatin is not significant between GIGT group and GDM group (P>0.05).2. Between 37 and 40 weeks,fasting plasma visfatin levels were increased, but that in the GDM group and the GIGT group are still significantly reduced compared with controls(16.52±6.54 ng/ml,19.01±6.06 ng/ml vs 24.24±5.84 ng/ml,P<0.01).3. FPG,FIns,HOMA-IR in GDM group are higher than that in the control group(P<0.05)。HBCI in the three group,there is no difference,but has elevated tendency in the GDM group . TG,TC in GDM group are higher than that in the control group(P<0.05)。There are no difference about glucose and lipid index between the GIGT group and the control group(P>0.05)。4.Plasma visfatin level is positively correlated with HDL ( r=-0.275, P=0.006),and negatively correlated with IR,TG,FIns(r=-0.349,P=0.000; r=-0.264, P=0.010;r=-0.248,P=0.013).Multiple regression analysis show that TG,HDL entered the regression equation: Y=19.760-2.572XHOMA-IR-1.630XTG +4.386XHDL.Conclusion1. Plasma visfatin levels in gestational diabetes mellitus all decreese in different weeks, visfatin acrinia is associated with GDM happen.2. Gestational diabetes mellitus has high insulin resistance,high lipid,glucose and lipid metabolism exist in gestational diabetes mellitus.3.Plasma visfatin level is associated with glycose and lipid metabolism. Visfatin may contributes to the pathogenesis of gestational diabetes mellitus. |
