[Objective]1.To approach the effect of N-acetylcysteine(NAC) in acute pancreatitis.2.To inves-tigate the influence of NAC in acute necrotizing pancreatitis for the level of reduced glutathione(GSH).3.To approach the the therapeutical effect and mechanism of NAC'si nfluence for the pancreatic tissue's GSH level in acute necrotizing pancreatitis.[Materials and Methods]1.Animal grouping:36 male Sprague-Dawley rats were randomly divided into three groups:sham operated (SO)group, acute necrotizing pancreatitis group with normal sodium injection(ANP), acute necrotizing pancreatitis group with N-acetylcysteine inj-ection(NAC).Every group has 12 rats.2. Model preparation and index detection:Every Sprague-Dawley rat is fasting diet 12 hours preoperative and drinking water at will. After intraperitoneal injection n-arcotization, opening abdominal cavity by epigastric zone median incision, then the ch-oledochus and duodenum were revealed. We only car-ried and overturned intestinal canal in SO group.Then we injected normal sodium(0.7-ml/100g) through vena dorsa-lispenis after 5 minutes. The model of ANP was establi-shed by retrograde pancreatic injection of 5% sodium taurocholate(0.1ml/100g). Then we injected normal sodium(0.7 ml/100g) through vena dorsalis penis. The acute necrotizing pancreatitis model of NA C was established the same as the ANP, but we inje-cted 16.7% N-acetylcysteine (0.7ml/100g) through vena dorsalis penis instead.We executed all Sprague-Dawley rats after 6 hours and collected hydrogaster, arterial blood and pancreatic tissue separately. Determining the quantity of hydrogaster,serum amylase, pancreatic gland's wet and dry ratio, saturation of arterial blood oxygen and the level of reduced glutathione in pancreatic tissue. After tissue fixation, embedment, sect-ioning and stained with hemato xy lineosin then scored according to pathobiology.[Result]1.Pancreatic tissue histopathology and pathobiology score:both ANP group and NAC group appear to process of acute necrotizing pancreatitis, but the NAC group's necrosis degree was fairly lighter and the pathobiology score was lower than the ANP group(p<0.05).2. the oxygen saturation of blood:ANP group was obviously higher than SO gro-up (p<0.05); NAC grop was lower higher than SO group, but obviously higher than ANP group(p<0.05).3.the quantity of hydrogaster, serum amylase, pancreatic gland's wet and dry rati-o:ANP group was obviously higher than SO group(p<0.01); NAC grop was higher t-han SO group, but obviously lower than ANP group(p<0.05).4.The level of GSH in pancreatic tissue:every group's GSH level was arranged as SO group> NAC group> ANP group, ANC group was obviously higher than ANP (p<0.05).5.De-pendablity analysis:NAC group contrast with ANP group, the level of GSH in pancreatic tissue and saturation of arterial blood oxygen presented positive correla-tion, r= 0.427(p<0.05). the level of GSH in pancreatic tissue and quantity of hydroga-ster,seru-m amylase, pancreatic gland's wet and dry ratio, pathobiology score all presented negative correlation, coefficient of correlation was respectivly:r=-0.6 32, r=-0.526, r=-0.413,r=-0.658 (p<0.05).[Conclusion]The method's achievement ratio of establishing model by retrograde pancr-eatic injection of 5% sodium taurocholate is high and stabilization.When we intravenous injected NAC for acute necrotizing pancreatitis model rats, the le-vel of GSH in pancreatic tissue was obviously raising and this can identify th-at NAC may convert to GSH as the precurosor of GSH. So NAC has some therapeutical effect for acute pancreatitis model.In our experiment, the level ofGSH in pancreatic tissue in NAC group was increase clearly after 6 hour. GSH may delay progression and lighten severity degree of acute pancreatitis by participating inhibiting inflammatory mediators and oxyradical, improving pancreatic microcirculationdisturbance, inhibiting intracytoplasm calcium ion ove rloading mechanism.
