Objective To access the accuracy and the clinical value of Brilliance 256-slices Spiral CT in estimating the coronary artery disease. Methods To analyse retrospectively five hundred patients who were taken Brilliance 256-slices Spiral CT,thereinto 49 case taken selective coronary angiography in one month,then a contrast study was taken between two of them. To access the accuracy of Brilliance 256-slices Spiral CT in estimating the coronary artery disease byχ2-test; one hundred patients were divided in two groups randomly undergoing 256ICT,50 patients were taken prospective ECG-triggered axial scan and the other were taken retrospective ECG-gated helical scan.The heart rate was 68.8 beats per-minute (b-pm) for A group and 76 b-pm for B group, and both of them under went 256ICTCA by prospective and retrospective ECG-gated scan.The image quality of coronary angiography and the effective radiation dose of two groups were analysed by t-test and rank sum test.Results:Taking coronary angiography as gold standard, The sensitivity,specificity,veracity,positive and negative predictive value of Brilliance 256-slices Spiral CT in diagnosis of coronary artery stenosis (=75%) is 93.7%,96.6%,96.3%,78.7% and 96.7%.The sensitivity of Brilliance 256-slices Spiral CT in diagnosis of coronary artery stenosis (=50%) is 87.1%. The mean image quality score of the prospective and retrospective ECG-gated groups was no significant statistical difference between them (P>0.05) in low heart rate.The image quality is no difference between two groups.The image quality score show significant statistical difference between the two groups in high heart rate. The image quality between the two groups is obvious difference.The mean effective radiation dose of prospective retrospective ECG-gated groups were (3.75±0.33) mSv and (11.44±1.13) mSv, and showed significant statistical difference between the two groups (P<0.05). The mean effective radiation dose of prospective group was decreased by 67.3% than the retrospective one, so Prospective ECG-triggered axial scan technique cut down to large degree it's damage for human. Conclusion:coronary artery imaging by Brilliance 256-slices Spiral CT weren't almost suffer from heart rate, and it has a high accuracy in diagnosis of coronary artery disease, and can show clearly pathological position, narrow degree, stents, graft patency and so on, be regard as one of no-trauma better methods to diagnose coronary artery disease today. The effective radiation dose of prospective ECG-gated groups is fall obviously.so this technique san can become one preferred examine method for coronary artery imaging without symptom people who is in low heart rate.
