| Objective:To compare the visual function in eyes implanted with 3 different types of intraocular lens (IOL).(the new apodized diffractive AcrySof ReSTOR multifocal IOls,refractive Array multifocal IOLs,the monofocal AcrySof Natural IOls) and evaluate clinical effects of the AcrySof ReSTOR multifocal intraocular lens.Methods:Random clinical trial.108 patients age-related cataract (126 eyes) were divided into 3 groups:Acrysof ReSTOR group included 30 patients (36 eyes),Acrysof Natural group(MoIOL) included 28 patients (30 eyes),the Array group included 50 patients (60 eyes). Measurements were taken3 months after operation,including uncorrected distant vision acuity (UCDVA),uncorrected near vision acuity(UCNVA,best corrected distant vision acuity(BCDVA,best corrected near vision acuity (BCNVA),distant corrected near vision acuity (DCNVA),Intermediate Vision (40cm,60cm,80cm) and contrast sensitivity(CS),depth-of-focus. Spectacle dependence, unwanted visual symptoms and patient satisfaction were asked at 3 months by questionnaire.Results There was no significant differences in UCDVA,BCDVA,BCNVA when ReSTOR VS Natural and ReSTOR VS Array. UCNVA and DCNVA were significantly higher in the Acrysof ReSTOR than Natural and Array. Intermediate Vision (40cm,60cm,80cm) were significantly higher in ReSTOR than Natural,but no significant differences between ReSTOR and Array. No statistical difference was noted with the refraction (spherical error,cylindrical error) while ReSTOR compare with Array or Natural. There was no statistical difference between the range of the depth-of-focus curves,the peak and trough almost in the same position on the ReSTOR and Array curves.The ReSTOR group depth of focus was 5.88D. The Natural group depth of focus was 1.88D There is significant differences in depth of focus between ReSTOR and Natural. The monocular contrast sensitivity measurement was not statistically and significantly different in the ReSTOR and Array groups at each spatial frequencies. The monocular contrast sensitivity was lower in the ReSTOR group than Natural group at each spatial frequencies.But only at the highest test spatial frequency(18c/d), the contrast sensitivity between ReSTOR and Natural groups was statistically significantly different. Glare and halos were dominated in the ReSTOR and Array groups.No statistical difference was noted between ReSTOR and Array groups with the adverse symptoms. dverse symptoms such as glare and haloes were significantly more bothersome with the Acrysof ReSTOR multifocal IOLs than the Natural monofocal IOLs(P<0.05). Unwanted photicphenomena (glare,halos) were clinically aceeptable. The dependence on spectacles in near vision between ReSTOR and Natural groups was statistically significantly different.It was lower in the ReSTOR group than Natural group.But the dependence on spectacle was no significant differences betweenReSTOR and Array.Conclusions:.the the Acrysof ReSTOR multifocal IOL provide excellent full range of vision; effectively decrease the dependence of glasses; improve the visual quality after sugery. |
