To compare the effects of combination therapy with amlodipine and fosinopril administering at different time on blood pressure (BP) and circadian BP pattern in patients with essential hypertension.MethodThe patients who had uncontrolled BP after treatment of amlodipine or fosinopril were enrolled.40 patients with grade 1-2 essential hypertension were randomly divided into two groups. Group A:Amlodipine was given in the early morning and fosinopril at bedtime seperately; Group B:Amlodipine and fosinopril were given in single morning dosing. The clinic BP and 24-hour ambulatory blood pressure were determinated before and after 4 weeks of treatment.ResultAfter treatment, the 24-hour mean of systolic and diastolic blood pressure (SBP and DBP) were reduced in both groups. While subjects in group A showed a significant reduction in the average nocturnal SBP and DBP (P<0.05). Accordingly, the diurnal/nocturnal BP ratio significantly increased.Conclusion Compared with treatment administering two drugs in the morning, the treatment administering two drugs separately at different time can significantly decrease nocturnal BP, increase diurnal/nocturnal BP ratio and normalize circadian BP pattern. Thus, chronotherapy is very important for blood pressure control and the proper modeling of the circadian BP pattern.
