| ObjectiveUsing Flotrac to investigate hymodynamic changes infusing hydroxyethyl starch 130/0.4 or Plasma Lyte A before anesthetic induction of coronary artery bypass grafting.MethodsFifty ASAⅡ~Ⅲpatients undergoing coronary artery gypass grafting surgery were randomly divided into 2 groups(n=25 each):hydroxyethyl starch 130/0.4(Voluven, group V)and Plasma Lyte A (group A).A left radial artery catheter was inserted and connected to Flotrac under local anesthesia,meanwhile a right internal jugular vein was punctured and was used to measure CVP.Patients in group V were infused 5ml/kg hydrooxyethyl starch 130/0.4 in 30min,group A were infused Plasma Lyte A 5ml/kg in the same time.Patients were induced with midazolam0.02mg/kg,sulfentanyl 0.8ug/kg, lidocaine 1mg/kg,rocuronium0.6mg/kg and etomidate 0.15mg/kg.Heart rate (HR) mean arterial pressure(MAP),systolic blood pressure (SBP),diastolic blood pressure (DBP),central venous pressure (CVP),stroke volume(SV),cardiac output (CO),systemic vascular resistance (SVR)were determined at the time when the patients were calm(T1),volume therapy (T2),before intubation after anesthetic induction(T3),30 seconds after intubation(T4),5min after intubation(T5). Statistical analyses were performed with the software package SPSS13.0.ResultsThere were no statistically significant differences in two groups in physical status,genger,age,weight,ejection fraction(EF), et al.P) 0.05.There were no statistical difference in baseline of both groups.the decrease of MAP,SBP,SV,CO were lower in group V than in group A.P (0.05.There were no statistically significant differences of two groups in CVP,HR and SVR.In group V,MAP,SBP,DBP,CO were significantly fall at T3and T5 comparing with T1.The same changes in group A.ConclusionInfusing colloid Voluven 5ml·kg-1 before anesthetic induction in OPCABG surgery is safe with patients aged younger than 75ys and EF) 40%.Comparing with infusing crystalloids Plasma Lyte A,the decrease of MAP,SBP,CO were lower ingroup V. |
