BACKGROUND:As the rapid development of endovascualr technology and instrument,more and more aortic diseas can be treated by EVR.However,commercially available devices which design for ascending aorta and aortic arch are still limited.Our reseach-team designed the"modular branched stent-graft system"to reconstruction ascending aorta and arch,and have finished animal-experiment already,and have finished the research about the size-design of the modular branched stent-graft by CTA. But lack of the elastic properties of the ascending aorta in human.So our object is patient in high-risk liability to aortic disease.Our aim is to design the "modular branched stent-graft system"which suitable for most part of the chinese population,and to provid datas for future production.AIM:Record and statistical analysis the elastic datas about ascending aorta and descending aorta in high-risk patient liability to aortic disease. To design the "size of modular branched stent-graft system"which suitable for most part of the chinese population,and to provid datas for future production.METHODS:45 patients with aortic disease were choosed to make DSA explorations of the thoracic aorta and measure the central blood pressure carried out between Sep.2007and Sep.2008 at our institution.We measured the maximum diameter and minimum diameter of ascending aorta(AA) and descending aorta (DA) in Si Chang work station. and calculate the change of diameter,aortic stiffness and distensbility Set up data base about all the measurements,we use the SPSS statistical software to analysis datas.RESULTS:The aortic maximum diameter above Coronary Artery(CA) (D1), the level at origin of BrachioCephalic Trunk (BCT)(D3), the halfway of the AA(D2), and the level at distal of origin of LSA(D4) respectivly are:35.53±4.94mm,37.48±5.32 mm,35.54±4.96 mm,26.95±3 mm;The aortic minimum diameter from D1 to D4 respectivly are:33.2±4.83mm,35.88±5.32mm,33.89±4.91mm,25.96±2.93mm;The change of aortic diameter from D1 to D4 respectivly are: 2.34±0.95mm,1.6±0.71mm,1.65±0.68 mm,0.99±0.28mm;The aortic stiffness from D1 to D4 respectivly are:9.67±5.02,15.54±7.85,13.78±6.45,15.53±4.74;The aortic distensibility (mmHg-1)×10"3 from D1 to D4 respectivly are: 2.76±1.65,1.76±1.15,1.94±1.23,1.33±0.40.The relative value of change of aortic diameter to the aortic minimum diameter from D1 to D4 respectivly are:7.18±3.21,4.6±2.3,4.96±2.22,3.86±1.16. The tapered angle(°) D2-D3 is:2.47±1.80.There are significance of the difference in aortic maximum diameter,aortic minimum diameter, change of aortic diameter,aortic stiffness,aortic distensibility(mmHg-1)×10-3 and the relative value of change of aortic diameter to the aortic minimum diameter from D1 to D4.Differed significantly about the aortic maximum diameter and aortic minimum diameter in the D1 and aortic stiffness,aortic distensibility (mmHg-1)×10-3 in the D4 between groups men and women. There are significance of the difference in aortic distensibility (mmHg-1)×10-3 in D2 in the person whose age weather arrive at 60.CONCLUSION:DSA is a precise way to precision measurement the change of aortic diameter during the cardiac cycle,and central blood pressure a precise way to precision measurement the change of aortic pp.we can calculate the aortic stiffness and aortic distensibility (mmHg-1)×10-3 which reflect the aortic elastic properties.The area of D2 is the most easily place to take place elastic decrescence at first,and the area of D1 is the most difficult place to take place elastic decrescence,it seems that the area of D1 can keep the elastic for more time.
