Relationship Between Plasma Omentin-1, Visfatin And Atherosclerosis In Newly Diagnosed Type 2 Diabetes Patients

Abstract:Objective To determine the plasma omentin-1 and visfatin levels of newly diagnosed type 2 diabetes patients with or without artery atheromatous plaques, and to explore the relationship between plasma omentin-1,visfatin level and diabetes with atherosclerosis.Methods 66 newly diagnosed type 2 diabetes subjects were choosen and divided into artery atheromatous plaques group(AAP group, n=34) and no artery atheromatous plaques group(nAAP group, n=32) by artery ultrasonography of carotid,femoral and iliac artery.The glycosylated hemoglobin Alc (HbAlc) and fasting insulin (FINS) of each group were determined,homeostasis model assessment of insulin resistance (HOMA-IR) was caculated. Another 32 healthy subjects were included as normal controls. All subjects were excluded severe inflammation, heart failure, cerebral vascular accident, cancer, immune system diseases and history.The height, weight, body mass index (BMI), waist circumference (WC), hip circumference (HC), systolic blood pressure (SBP), diastolic blood pressure (DBP), fasting blood glucose (FBG), triglyceride (TG), total cholesterol (TC), low density lipoprotein (LDL-C), high density lipoprotein (HDL-C) were detected in all three groups,The Enzyme-linked immunosorbent assay (ELISA)technique was used to determine the plasma omentin-1 and visfatin. Application SPSS 17.0 statistical software for data processing, analysis of variance or t test was used to show the different between the three groups,.simple correlation (Pearson correlation) and stepwise regression analysis were used to show the relationship of moentin-1 and visfatin between the other indicators. The difference was statistically significant if P<0.05.Results For age and gender,there were no significant difference Among the three groups.The concentration of plasma omentin-1 and high density lipoprotein (HDL-C) were decreased significantly in type 2 diabetes mellitus group compared with those in the control group(P<0.05). The concentration of plasma visfatin,the systolic blood pressure (SBP), diastolic blood pressure (DBP), body mass index (BMI), waist circumference (WC), hip circumference (HC), fasting blood glucose (FBG), low density lipoprotein (LDL-C),was increased significantly in type 2 diabetes mellitus group compared with those in the control group(P<0.05).The waist hip ratio (WHR) of the type 2 diabetes mellitus group and the control group was no significant difference (P> 0.05).The concentration of plasma omentin-1, triglyceride (TG) and fasting blood glucose (FBG) were decreased significantly in AAP group compared with that in nAAP group(P<0.05). The systolic blood pressure (SBP) was increased significantly in AAP group compared with that in nAAP group(P<0.05). The diastolic blood pressure (DBP), body mass index (BMI), waist circumference (WC), hip circumference (HC), waist hip ratio (WHR), glycosylated hemoglobin Alc (HbA1c), total cholesterol (TC), high density lipoprotein (HDL-C), low density lipoprotein (LDL-C), concentration of plasma visfatin, fasting insulin (FINS) and homeostasis model assessment of insulin resistance (HOMA-IR) in the AAP group were no significant difference compared with that in nAAP group (P>0.05).Plasma omentin-1 level was negatively correlated with low density lipoprotein cholesterol(LDL-C),wight,body mass index(BMI),waist circumference(WC),hip circumference (HC) and the natural logarithm of fasting blood glucose[ln(FBG)] (P<0.05). Plasma visfatin level was positively correlated with age, the natural logarithm of fasting insulin[ln(FINS)],low density lipoprotein cholesterol (LDL-C),the natural logarithm of insulin resistance index[ln (HOMA-IR)] and the natural logarithm of fasting blood glucose ln(FBG) (P<0.05). There was no significant relationship between omentin-1 level and visfatin level (P> 0.05).In diabetic patients,with omentin-1 as the dependent variable,and visfatin,low density lipoprotein cholesterol(LDL-C),age,hight,weight,wight,body mass index(BMI),waist circumference(WC),hip circumference (HC),waist hip ratio (WHR),systolic blood pressure (SBP),diastolic blood pressure (DBP),natural logarithm of fasting blood glucose[In(FBG)],natural logarithm of fasting insulin[ln(FINS)],natural logarithm of insulin resistance index[ln (HOMA-IR)],total cholesterol (TC),natural logarithm of triglyceride [In (TG)],high density lipoprotein (HDL-C),glycosylated hemoglobin Alc (HbAlc) as independent variables,the multiple stepwise regression analysis was carried out.The result show that natural logarithm of triglyceride [In (TG)],low density lipoprotein cholesterol(LDL-C) and body mass index(BMI) are the factors independently associated with omentin-1. (P<0.05)In diabetic patients,with visfatin as the dependent variable,and visfatin,low density lipoprotein cholesterol(LDL-C),age,hight,weight,wight,body mass index(BMI),waist circumference(WC),hip circumference (HC),waist hip ratio (WHR),systolic blood pressure (SBP),diastolic blood pressure (DBP),natural logarithm of fasting blood glucose[ln(FBG)],natural logarithm of fasting insulin[ln(FINS)],natural logarithm of insulin resistance index[ln (HOMA-IR)],total cholesterol (TC),natural logarithm of triglyceride [ln (TG)],high density lipoprotein (HDL-C),glycosylated hemoglobin Alc (HbAlc) as independent variables,the multiple stepwise regression analysis was carried out.The result show that natural logarithm of insulin resistance index[ln (HOMA-IR)] is the factors independently associated with visfatin. (P=0.011)Conclusion Plasma omentin-1 is closely related to type 2 diabetes mellitus and plasma omentin-1 is significantly related to the occurrence and development of the atherosclerosis of early type 2 diabetes mellitus.Omentin may play an important role in the occurrence and development of the disorder of the grease metabolizes,metabolic syndrome, type 2 diabetes mellitus and great vessels complication of diabetes.Plasma visfatin is closely related to type 2 diabetes mellitus.visfatin is not significantly related to the occurrence and development of the atherosclerosis of early type 2 diabetes mellitus.visfatin may play an important role in the occurrence and development of the disorder of the grease metabolizes,metabolic syndrome and type 2 diabetes mellitus.More studies should be done about the influence of the visfatin to great vessels complication of diabetes,special the development of the atherosclerosis.
