Objective: To compare the advantages and disadvantages of the Three-dimensional CT angiography and two-dimensional digital subtraction angiography (2D-DSA) in the diagnosis of intracranial aneurysms.Methods: 3D-CTA and 2D-DSA were performed in 41 patient's cases of suspected intracranial aneurysm form March 2009 to April 2010 in Neurosurgery of The First Affiliated Hospital of Guangxi Medical University. 19 cases underwent 3D-CTA before 2D-DSA, 22 cases were performed 2D-DSA before 3D-CTA scan. Images of 3D-CTA and 2D-DSA were blindly reviewed by two independent radiologists and neurosurgeons. Two Imaging Technology text results were recorded and comparative study.Results:All of the 41 patients .37 intracranial aneurysms were found in 34 cases by examination and clinic treatment,7 patients had no aneurysm.34 patients had single aneurysm,3 patients had 2 aneurysms.36 aneurysms were detected by 3D-CTA in all 33 patients,missed diagnosis 2 aneurysms(1single aneurysm, 1 in multiple intracranial aneurysms),1 false-positive. DSA demonstrated 36 aneurysms in 33 patients, missed diagnosis 1single aneurysm, no false-positive. The sensitivity of CTA for the detection of all aneurysms was 94.6%,the specificity85.7%,the accuracy 93.2%.For the DSA ,the sensitivity , specificity , accuracy were 97. 7 % , 100 % , 97. 3 %.In 3D-CTA and 2D-DSA accuracy comparison ,there was no significant difference(p>0.05),However, there was statistical significances in neck width survey (P<0.05)Conclusion: Both of the 64-Section 3D-CTA and 2D-DSA have very high diagnostic value for intracranial aneurysm. 64-section 3D-CTA is a safe,economical and noninvasive method for the diagnosis of intracranial aneurysms and has highly sensitivity and accuracy,which can be used as first choice for diagnosing intracranial aneurysms.
