| Objective To research anatomic variations and incidence of the hepatic arterial in HCC patients by digital subtraction angiography(DSA), and combine with documents to investigate the clinical significance.Materials and Methods Retrospective analysis hepatic artery DSA material of 500 cases of liver cancer patients,and observe the origins and courses of hepatic arteries. The hepatic artery outside of the Michels I type classified as variation hepatic arteries and determine their types. Calculate the incidence of each variation type. Analysis the various types'variation and its distribution characteristics by using statistical methods to contrasted our group with Michels autopsy group and two domestic group for finding out whether these data are consistent. Summarizing the distribution of the hepatic arteries' openning position and the corresponding treatments methods.Results The standard hepatic arterial anatomy was seen in 82.6% and the arterial anatomy was anomalous in 87 out of 500 cases with an overall incidence of hepatic artery anomalies of 17.4%.60 of the cases fitted to Michels classifications,occured in 12% of cases.2 cases(0.4%) belonged to Michelsâ…¡type,43 cases(8.6%) belonged to Michelsâ…¢type,3 cases(0.6%) belonged to Michelsâ…¤type,1 cases(0.2%) belonged to Michelsâ…¥type,11 cases(2.2%) belonged to Michelsâ…¨type, and other Michels'classifications was not be found.27 of the cases were out of the Michels classification which accountted 5.4%. There is poor consistency of the various types'variation and its distribution between our groups and the others.58 variation hepatic artery give tumor blood directly.53 cases are the right hepatic artery and 5 cases are the left hepatic artery.20 cases accepted TACE treated and 1 cases accepted liver transplantation.Conclusion The hepatic artery anatomical variations with complexity and diversity, there are also some randomness. Michels classification cannot fully generalization of hepatic artery. It is important to understand hepatic artery and its classification of variation which has important guiding significance in clinical surgery and liver interventional therapy. |
