0bjective To investigate the response rate and side-effects of Pemetrexed and Docetaxel as a second-line regimen in the therapy of advanced non-small cell lung cancer in elderly patients by Meta-analysis Methods Aftergenerally collection the clinical literatures published in domestic and overseas from 1994 to 2011 that reported on the response rate and side-effects of Pemetrexed and Docetaxel in the therapy of advanced non-small cell lung cancer in elderly patients Meta-analysis was performed by Review Manager 5.0 software. Fixed or random model based on the heterogeneity test are uesed Results Two literatures were entrolled for Meta-analysis based on including and excluding creterias Two RCT studies in in elderly patients groups one year survival rate between pemetrexed group and the docetaxel group is the RR = 0.94 95% confidence interval [0.50 1.77] test for overall effect Z = 0.20 P = 0.84 the difference was not statistically significant overall response rate (disease control rate) group RR = 1.18 95% CI [0.861.60] test for overall effect Z = 1.03 P = 0.30 the difference was not statistically significant Grade - neutropenia [RR = 0.42; 95% CI [0.171.04]; Z = 1.87 P = 0.06. Pemetrexed group showed less neutropenia ratedifferences were statistically significant.There was no diference between the response rate between Pemetrexed and Docetaxel but decrease of neutrophils was greater in Docetaxel regiment Conclusion In elderly patients with advanced non-small cell lung cancer Pemetrexed and Docetaxel both are effective with on significant differences in one-year survival rate and response rate.Decrease of neutrophils is greater in Docetaxel regiment but it should be tested for further investigation. Sensitivity analysis and publication bias in the recognition results show that meta-analysis of this result stable and reliable.
