Objective:This study attempts to detect percentage of NKG2D positive cells in peripheral blood CD8+NKT cells and their secreted ligand expression sMICA of esophageal carcinoma and cardiac carcinoma, and to analysis of two clinical significance abounding clinical pathological, and the compliance between both of them and the tumor immune escape mechanism in patients, and the clinical significance of both of them as indicators of surgical treatment effect. Method:53 patients with esophageal and cardiac carcinoma, 29 postoperation patients,30 normal subjects, using flow cytometry to detect the percentage of NKG2D positive cells in peripheral blood CD8+NKT cells and their secreted ligand expression sMICA and enzyme-linked immunosorbent assay (ELISA) to detect serum levels of sMICA.Result:The peripheral blood NKG2D positive CD8+NKT cell percentage in patients was significantly lower than that in controls ([77.632±1.232)%(,89.053±1.189)%]( P<0.05); with stageⅡ,Ⅲ,Ⅳ, it decreased significantly in orde(rP<0.01); with lymph node metastasis lower than that without lymph node metastasis(P<0.01); imbricate carcinoma was higher than glandiform carcinoma (P<0.01); after operation was significantly higher than before operation(P<0.01). The level of serum sMICA in patients was significantly higher than that in controls[(326.28±11.732)pg/ml,(210.00±4.119)pg/ml](P<0.01); with stageⅡ,Ⅲ,Ⅳ,it increased significantly in order(P<0.01); with lymph node metastasis higher than that without lymph node metastasis(P<0.01); imbricate carcinoma was lower than glandiform carcinoma(P<0.01); after operation was significantly lower than before operation(P<0.01). Serum sMICA can inhibit peripheral blood CD8+NKT cell activation receptor NKG2D(P<0.05), determination coefficient R2=0.7368.Conclution:the percentage of NKG2D positive cells in the peripheral blood CD8+NKT cells decreased and the expression of serum sMICA increased in Esophageal and cardiac carcinoma patients . with the former decreased the latter increased ,the latter can inhibit the former,which accounts for 73.68%. The change of Both of them are related to immune escape mechnasim in esophageal and cardiac carcinoma patients. It is significant to detect the lever of both of them to as the auxiliary diagnosis of clinical stage, and to determine The biological behavior and prognosis of esophageal and cardiac carcinoma , and indicators of surgical treatment effect. This study further confirmed that both of them play an important role in immune escape mechanism in esophageal and cardiac carcinoma patients, and providing a new idea in immunotherapy of esophageal and cardiac carcinoma patients.
