objective:To investigate whether lipocalin -2 can determine prognosis of acute pancreatitis by comparing the levels of lipocalin -2 (LCN2) in peripheral blood between normal healthy adult, mild acute pancreatitis patients, severe acute pancreatitis patients Methods:1,44 patients whom the onset of acute pancreatitis within 24 hours consisted of a case group according to the diagnostic criteria for clinical guidelines of acute pancreatitis of china in 2004. The case group divided into mild group (M group) and sever group (S group) according to disease process of the patients.32 cases of healthy adults randomly selected as normal control group (N group).2, the gender, age and the degree of obesity (indicated by body mass index) were recorded in control group. APACHEⅡscore was executed in case group within 24 hours and 24h to 48h after admission. The blood samples within 24h and 48h after the onset were received in the case group and in the control group. Serum specimens centrifuged from blood samples, were stored in-70℃. The C-reactive protein of the same patient also was detected at the same time segment.3, The Enzyme-linked immunosorbent assay was used to test the value of lipocalin -2 of serum specimens received at different times both in the control group and the case group.4,1). The age, gender, body mass index were compared between N group, M group and S group.2). LCN2 values were compared between N group, M group, S group. The value of C-reactive protein and LCN2 also were compared between M group and S group.3) The LCN2 and CRP levels in S group and M group according to the time of collection of the sample were compared.5, all results were analyzed by SPSS 19.0 software. Results:1,N group (33.37±6.62), M group (39.00±8.45) and S (39.78±10.72) were not differ between age, all P> 0.05.2, The gender composition (male/female) were 18/14,18/8,10/8 in N group, M group, S group, respectively.The test did not differ, all P> 0.05.3,The body mass index (22.90±1.87, Kg/m2) of N group was different with the M group (24.71±2.63, Kg/m2)and the S group(25.58±2.33, Kg/m2), P <0.05. Body mass index of mild group and severe group did not differ, P> 0.05.4, The value of LCN2 in N group (83.16±28.47,μg/L) was lower than the value of M group (106.36±17.84,μg/L, P<0.05) and S group (658.96±280.60,μg/L, P<0.05). The value of LCN2 of serums samples collected within 24h after onset between the M group and S group were different,P<0.05. The value of LCN2 of serums samples collected between 24h and 48h after onset between the M group and S group also were different, P<0.05. The value of CRP within 24h after onset between M group (5.32±2.72, mg/L) and S group (6.38±2.21, mg/L) were no different, P> 0.05. The value of CRP,tested between 24h and 48h after onset, between M group (25.27±38.85, mg/L) and S group (58.85±33.19, mg/L) were difference, but not very significant, P=0.048<0.05.5, The values of LCN2 and CRP at 24h with the value of LCN2 and CRP at 48h were both not different in M group, P> 0.05. In S group, the values of LCN2 at 24h with the value of LCN2 at 48h showed no difference, P> 0.05.but the value of CRP showed difference between at 24h and 48h in S group. P<0.05. Conclusion:1, obesity is the predisposing factor of acute pancreatitis, maybe one of the indicators of poor prognosis.2, LCN2 can be used as an indicator to determine the prognosis of acute pancreatitis。...
