Objective To compare the differences of 13 common clinical risk factors between cerebral infarction patients with and without leukoaraiosis(LA), and to investigate the potential etiology and pathogenesis of LA.Methods 193 patients hospitalized in the neurological department of First affilicated hospital of Shanxi medical university were randomly collected from Septemper of 2010 to March of 2011.A11 the patients undergone cranial CT or MRI. All cases were divided into group LA(LA,87 cases)and groupNLA(LA+CI,106 cases) according to the diagnostic standard of LA that wrote by Guo hongzhi in 2002. Clinical data of the two groups, such as age, sex, smoking history, alcohol history, hypertension history, diabetes, coronary heart disease, plasm TG,LDL,HCY,HsCRP,the stenosis extent of carotid atherosclerosis, unstable carotid plaque, were noted. Then Single factor analysis and Binary-Logistic regression analysis were performed.Results The incidence of LA was 45.1% in 193 patients with cerebral infarction. In univariate analysis, the two groups had significant differences in age, HCY, HsCRP, hypertension, diabetes, the stenosis extent of carotid atherosclerosis and unstable carotid plaque(P<0.05), then had no statistical significance in other factors such as plasm TG,LDL,sex, smoking history and alcohol history. In binary logistic regression analysis, older age, hypertension, coronary heart disease and unstable carotid plaque were independent risk factors of LA, but HCY, HsCRP, diabetes and the stenosis extent of carotid atherosclerosis (β=-1.034)were not independent risk factors of LA.Conclusions Old age, hypertension, coronary heart disease and unstable carotid plaque are involved in the pathogenesis of LA and are independent risk factors of LA. HCY, HsCRP and diabetes promote the occurrence of LA, but are not independent risk factors of LA, probably playe a role in the occurrence and development of LA with other factors. Stenosis of carotid atherosclerosis have a protective effect on cerebral small vessels. It is necessary for LA patients to assess and interfere cardiovascular status and degree of carotid artery stenosis and unstable carotid plaque by electrocardiogram(ECG) or coronary angiography and carotid artery ultrasound. And we should control blood pressure, glucose, HCY and HsCRP positively. It has a positive clinical significance for the occurrence and development of LA.
