| Purpose: To explore quantitative ~1H MRS analysis of fatty liver,provide a noninvasive method for the relatively quantitative inspection of the disease, and assist clinical diagnosis, treatment and follow-up.Materials and methods: Magnetic resonance imaging (MRI) and Magnetic resonance spectroscopy (MRS) data of 49 cases of adult nonalcoholic fatty liver disease(34 male,14 female) confirmed by liver biopsy were retrospectively analyzed. Blood tests were undergone in all of the 49 cases for the content of triglyceride, blood glucose, and total cholesterol, then went through conventional MRI and MRS spectrum analysis inspection using a SIEMENS 3.0T magnetic resonance scanner. Within one week after MRI examinations, liver biopsy were performed on the tissue sample of all cases to classify as F0, F1, F2, F3 or F4. The same region of interest were examined twice using ~1H MRS with and without water suppression sequence to the water area without water suppression sequence were calculated and then applied for the statistical analysis of histopathological classification result using the ROC curves.Additonally, pathologically classified lipid area percentages in these fatty liver cases were statiscally analyzed along with other indexes such as triglycerides indexes, glycemic index, and total cholesterol indexes respectively.Result:1.The averages of the lipid-water ratios for F0, F1, F2, F3, F4 are in percentages,0.77%, 2.12%,13.12%,20.20%,repectively.2.Application H-test (H =40.89)indicates that the lipid-water ratios of F0, F1 , F2, F3, F4 are different. This difference is statistically significant. 3.According to the ROC curves, the threshold value of lipid-water ratios in these cases of fatty liver disease is 1.8%, Therefore, the sensitivity value is 89.2%,the specificity value 91.7%,the positive predictive value is 0.92,the predicted negative value is 0.73.4.The correlation coefficient of classified lipid-water ratios average to Spearman rank correlation is 0.919(P<0.05),which is highly linear. The correlation coefficient of lipid-water ratios average to triglycerides index is 0.413 (P<0.05),which is significantly related, The correlation coefficient of lipid-water ratios average to glycemic index is 0.125 (P > 0.05), The correlation coefficient of lipid-water ratios average to total cholesterol index is 0.212 (P > 0.05).Conclusion:1.Lipid-water ratios of fatty liver disease by ~1H MRS spectrum and histopathological classification are significantly correlated.2.Quantitative analysis of ~1H MRS spectrum is an important non-invasive method for the diagnosis and classification of fatty liver disease. |
