Objective:To evaluate left ventricular regional diastolic and systolic function in patients with hypertrophic cardiomyopathy (HCM) by echocardiography.Methods: 40 patients with hypertrophic cardiomyopathy and 40 healthy subjects were performed by pulsed wave Doppler (PWD) ,TDI, strain (S) and strain rate imaging (SRI). The atrioventricularva1ves inflow velocity (E wave and A wave) by PWD and the atrioventricularva1ves annulus motion (including Em wave, Am wave from six sites including posterospetal, lateral, anterior, inferior, posterior and anteroseptal) by TDI were measured, the systolic peak S were measured at each segment of the left ventricular in all patients with hypertrophic cardiomyopathy and healthy subjects. E/A, Em/Am and E/Em were calculated. The differences in variables and the evolution of Em with gestation were analyzed.Results: Em, Em/Am from all sites in patients with hypertrophic cardiomyopathy decreased significantly. In patients with hypertrophic cardiomyopathy, Em Em/Am decreased significantly from anteroseptal site decreased significantly. E/Em in patients with hypertrophic cardiomyopathy increased significantly. The systolic peak S of each segment of the left ventricular decreased significantly when compared with healthy subjects, only middle segment and base segment of anteroseptal base segment of posterospetal and middle segment of inferior decreased significantly when compared with each other in patients with hypertrophic cardiomyopathy. Em was negatively related to wall thickness, Em decreased with wall thickness increasing. Conclusions:1. S can be used in the evaluation for left ventricular regional systolic function in patients with hypertrophic cardiomyopathy (HCM) reliably. Em, Em/Am and E/Em were good index of the evaluation for left ventricular regional and global diastolic function in patients with hypertrophic cardiomyopathy(HCM). 2. Em and S were negatively related to wall thickness.
