ObjectiveTo investigate the clinical value of narrow band imaging(NBI)in diagnosis of Superficial Esophageal Carcinoma and precancerous lesions.Methods34 patients with esophageal mucosa roughness, erosion, laque, bnormal color and indentation in conventional endoscopy and 10 cases of healthy physical persons were erolled. Endoscopy was performed with a conventional endoscopy, a magnifying endoscopy, a narrow-band imaging endoscopy,and a Chromoendoscopy(Lugol's iodine staining).Then we carefully observed the outline of lesions and the intrapapillary capillary loop(IPCL) and evaluated the image clarity.the biopsy samples were taken in suspicious area and patients with malignant lesions were treated by endoscopic mucosal resection or surgical operation. Seven patiens underwent preoperative ultrasonic endoscope examination. The comparion among the endoscopic findings,EUS and histological results was evaluated.Result(1)Visualization of silhouette of esophageal lesions by NBI endoscopy and chromoendoscopy were clearer than the conventional endoscopy.And there was no significant difference between NBI endoscopy and chromoendoscopy. (2)Visualization of the intraepithelial papillary capillary loop (IPCL) by NBI combinded with magnifying endoscopy was clearer than the magnifying endoscopy.(3)90% normal people corresponded to type I; 86.7% sophageal inflammations corresponded to Typeâ…¡; 85.7% mild atypical hyperplasia corresponded to Typeâ…¢;100% severe dysplasia corresponded to Type IV; 83.3% intramucosal carcinoma corresponded to Type Vm; 75% submucosal carcinoma corresponded to Type Vsm.(4)Seven patiens underwent preoperative ultrasonic endoscope examination. The intramucosal carcinoma accuracy rate of NBI combinded with magnifying endoscopy was 100%, higher than the ultrasonic endoscope accuracy rate, but there was no difference for submucosal carcinoma accuracy rate between them.ConclusionAs a new technique, NBI is not only very esay to operated, but only can capture optimal images of the early esophageal carcinoma's lesions and show better the IPCL. NBI may help predict in vivo histologic diagnosis of esophageal pathologic conditions with a good degree of accuracy, and it is always helpful for improving the accuracy of Superficial Esophageal Carcinoma and precancerous lesions on endoscopic target biopsies.
