OBJECTIVE:To evauate the changes of the serum and urinary Cystatin C(Cys C) in primary nephritic syndrome and the clinical significance of Cys C.METHODS:Serum and urinary samples were obtained from 30 cases with primary nephritic syndrome. The control group contained 30 healthy persons. Serum and urinary Cys C were determined by Particle-enhanced turbidimetric innunoassay (PETIA) and compared the statistically significant difference. Serum creatinine(Scr), blood urea nitrogen (BUN) and urinary protein were determined at the same time. Used Modified glomerular filtration rate estimating equation for Chinese patients with chronic kidney disease calculated the estimated glomerular filtration (eGFR). The correlation between eGFR and serum Cys C,Scr BUN was compared, the area under the Receivr Operating Characteristic Curve(AUCROC) and the Likehood Ritio(LR) were used for evaluating the reliability of serum and urinary Cys C in primary nephritic syndrome.RESULTS:Serum and urinary Cys C were significantly higher than control group's (P<0.05), while Scr and BUN were no statistical difference between cases and control group (P>0.05). The significant negative correlation between eGFR and serum Cys C was found (P<0.05), the correlation coefficient was-27.012。But The significant negative correlation between eGFR and Scr BUN was not found.CONCLUSIONS:1. Serum Cys C maybe is a more sensitive and reliable parameter of renal dysfunction than Serum creatinine and blood urea nitrogen in early primary nephritic syndrome.2. Urinary Cys C maybe reflect the tubular injury of the primary nephritic syndrome.
