Gitelman Syndrome: Report Of 4 Cases

ObjectiveTo summary 4 cases and to review the literature of Gitelman syndrome to discuss the pathogenesis,clinical manifestations.diagnosis and treatment in order to have patients of such disease diagnosed and treated earlier in the future.MethodsClinical datas of 4 cases of Gitelman syndrome admitted in the 1st affiliated hospital of Medical College, Zhejiang University among 2009-2011 were analyzed retrospectively.ResultsThe age of onset was adult,3 patients had varying degrees of weakness and tetany,while 1 had no obvious symptoms,Biochemical tests showed hypokakemic.hypomagnesaemia.hypocalciuria,metabolic alkalosis and renin-angiotensin-aldosterone system was actived with normal or low blood pressure.Serum potassium and magnesium levels raised up and the symptoms relieved after treatment of potassium and magnesium supplementation,spironolactone and ect.ConclusionAdults have weakness, cramp and biochemical tests suggest hypokakemic,hypomagnesaemia,metabolic alkalosis with normal or low blood pressure should be considered of such disease.Treatments include potassium and magnesium supplementation, aldosterone antagonists, NaCl transport blockers on distal convoluted tubule, prostaglandin synthase inhibitors and etc.Generally,the prognosis is good.
