Objective: To observe the clinical value of CEA in blood and bronchoalveolar lavage fluid for diagnosis in patients with lung cancer.Methods: 50 cases of lung disease were studied.Blood samples were taken from all of the patients at admission.BALF of the patients were gathered.Levels of CEA in blood and BALF were measured by radioimmunoassay method.With SPSS15.0 soft wrap,nonparametric test were used to analyze the data. There is a statistic significance when P<0.05.Results: All specimens (BALF and blood) were obtained from FUZHOU general hospital.Lung cancer group consisted of 28 cases,22 males and 6 females,age 48-74y,mean age(61. 1士10.8)y,was divided into two groups,nonsmall cell lung cancer(NSCLC) and small cell lung cancer(SCLC),based on histology.NSCLC consisted of 26 cases,including 12 cases of squamous cell carcinoma,14 cases of adenocarcinoma;SCLC consisted of 2 cases.Clinical staging: In accordance with AJCC/UICC,S TNM system(2002).Benign pulmonary disease consisted of 22 cases, 12 males and 10 females;age 28-76y,mean age (57. 0土20. 3)y, including 6 cases of chronic obstructive pulmonary diseases,10 of pneumonia,3 of bronchiectasis,3 of benign tumor of lung.Whether in blood and BALF ,the levels of CEA in case of lung cancer were showed significantly higher than those in case of benign lung disease(p<0.05).The levels of CEA in BALF taken from the affected lungs,sides were higher than those in BALF taken from the opposite normal ones in patients with lung cancer(p<0.05).The levels of CEA in both side BALF in patients with benign lung disease were not significant difference (p>0.05).The levels of CEA in BALF and blood in Adenocarcinoma were higher than those in Squamous cell carcinoma(p<0.05) .The cut-off value of BALF-CEA/serum-CEA is 9.The sensitivity,specificity is 57.14%,86.36%,respectively,The positive rate of detecting both the CEA of blood and CEA of BALF/CEA of blood was 85.71,which was higher than that of detecting either the CEA in blood or cytology in BALF. Conclusion: It is singnificant to detect the levels of CEA in blood and bronchoalveolar lavage fluid.It can help us to distinguish benign and malignant lung lesions,adenocarcinoma and squamous cell carcinoma,and it can help us to improve the diagnosis of lung cancer.
