Objective The purpose of this study was to evaluate the accuracy of unipolar electrogram for identifying the effective ablation sites in patients with premature ventricular complex (PVC) arising from aortic sinus of Valsalva.Methods Nine patients who received CARTO system-guided ablation therapy for PVC arising from aortic sinuses of Valsalva were enrolled retrospectively.12-leads surface electrogram during PVC, bipolar and unipolar electrograms of the effective ablation site were recorded and analyzed.Results Local ventricular activation preceding the onset of QRS wave by 29±4.6ms were recorded in the effective ablation sites, "QS" waves were detected in unipolar electrogram in 6/9 patients, while "rS" waves were detected in the remaining 3 patients.Conclusion in patients with PVC originating from aortic sinuses of Valsalva, QS morphology in unipolar electrogram was an inadequate guide for the identification of the effective ablation sites, bipolar electrogram and pace mapping should be analyzed simultaneously.
