Objective To evaluate the clinical validity of Physiological and operative score for the enumeration of mortality and morbidity (POSSUM) in lung cancer patients undergoing surgery in order to get clinical treatment reference. Methods Physiological and operative severity scores were kept in 179 lung cancer patients undergoing surgery.The physiological score and the operative risk score were compared between the groups with or without the postoperative complications. The mortality and morbidity rates were calculated. Actual mortality and morbidity rates were compared with the rates predicted by POSSUM. And the 5 single factors related to the mortality and morbidity risk were analyzed. Results Among 179 patients, there were postoperative complications in 78 patients. The physiological score and the operative severity score were significantly higher in the group in whom complications occurred compared with those without complications (16.11±2.53 vs 14.88±1.86 for physiological score, P<0.05; 13.47±2.83 vs 12.88±2.57 for operative severity score, P<0.05). The morbidity rate predicted by POSSUM was (65/ 179) and the actual morbidity rate was (78/179). There was no significant difference (P= 0.161). The mortality rate estimated by POSSUM was (12/179), which was significantly different from the actual rate (3/179) (P=0.018). Single factor analysis revealed that all 5 single factors have the relationship to the complications,and 1 single factors have the relationship to the death. Conclusion The POSSUM gave satisfactory prediction of morbidity rate but overrated the prediction of mortality rate in lung cancer patients undergoing surgery,and 5 single factors of the operation risk showed a better clinical value.
